### Job summary

Due to internal promotion Andover and Winchester ICTs are looking to recruit full time Community Charge Nurses. The Integrated Community Team model aspires to provide a responsive, safe effective caring service that is well led. The team is commissioned to reflect the needs of the local community. Service aims include providing a local point of access, proactive case management and adoption of a co-produced approach with patients and service users. The Integrated Community Team, whilst operating from a local hub, delivers a joined-up approach across a specific locality, to deliver excellence in care and engages with all practices and wider stakeholders in the locality.The primary role of the Community Charge Nurse is to provide strong holistic leadership to the team and support the Team Matron (Band 7)In conjunction with a range of clinical colleagues, will lead and facilitate a patient co-ordinated case management approach for people who are most vulnerable to, and at high risk of repeat admissions to hospitalWill participate in the holistic assessment, implementation, and evaluation of programmes of care for patients in the home, without direct supervision. This includes those with chronic disease, terminal illness who wish to die at home and complex care needsWill work across the caseload and the single point of access, utilising their clinical triage skills to identify the needs of patients, liaising with the correct services to ensure optimum care is delivered

Ensure new staff receive a proper induction so that they understand acceptable practice and standardsDevelop own professional skills and ensure these are shared with the team and wider divisionProvide management responsibility for the team in partnership with the Community Team Matron. This may include annual leave management, sickness/ absence management; performance management, training and development, appraisals, incident and complaints managementDeputise for the Integrated Community Team Matron as requiredBuilding and maintaining effective relationships with all stakeholdersEnsure effective management of own and team caseload
