Welwyn and Hatfield ntegrated Community Team are seeking motivated and enthusiastic Community Senior Nurse to join our friendly, supportive and Gold Accredited Team. You will be working alongside our nursing support staff, therapy staff and core nursing team to provide safe, effective, and high quality care to our patients within their own homes. We understand how important it is to have time to settle in to a new team; all staff joining our team are provided with a thorough 4-6 week induction period. Here, you will have the chance to work as a supernumerary member of the team, shadowing numerous staff in various roles within and outside of our team before working independently.

HCT will prioritise recruitment of workers living in England under the new immigration rules for care workers effective from 9 April 2025. ### Main duties of the job

Welwyn and Hatfield Integrated Community Team is an expanding and evolving service. We provide a wide range of nursing led care to people within their own homes. As a nurse in our team, you will be supported to develop and broaden your skill set. To achieve this, we facilitate our staff to access an extensive training and education programme, as well a range of personal and professional development opportunities including the PgDip Specialist Community Nursing course and prescribing courses. We also offer training in leadership and management to support you to manage your team effectively. In order to support your clinical and professional development, we provide regular supervision, held alternatively with a matron and manager.

Our service operates 8am – 10pm, 365 days a year. We can offer flexible working patterns and pride ourselves on working with our staff to ensure their work schedule supports a healthy work life balance. We are happy to consider part time applicants.

A full valid driving licence and access to a car to use regularly for business purposes is essential (unless you have a disability as defined by the Equality Act 2010).

### Detailed job description and main responsibilities

Our Community Nursing Sisters are committed to teamwork, have excellent communication and decision making skills and consistently work collaboratively with other professionals within HCT and beyond. Our Community Nursing Sisters regularly attend Gold Standard Framework meetings, to discuss care of palliative patients with a wider network of health care staff, and are often included in other MDT working such as safeguarding meetings and supporting the voluntary sector where possible. We aim to provide a high standard of patient care using an evidence-based framework.

Successful applicants will be responsible for assessment, planning, implementation, and evaluation of individualised nursing care to patients living in the community. You will manage a caseload of a smaller area within the locality, working with your team to oversee high quality care for all our patients. You will act as a role model for your team, to foster an environment of openness and honesty thus enabling excellent patient care.

A commitment to health promotion teaching and training is essential, to optimise our patient’s ability to self-manage their health needs and engage them in maintaining their wellbeing.
