We are looking for an ambitious, dynamic, motivated and enthusiastic Band 6 caseload holder or qualified District Nurse to join our Community Nursing team in Mid Beds. This highly respected team is based delivers responsive, high quality nursing to patients living in and around Biggleswade, Mid Beds. We are passionate about patient centred care, support our nurses to lead and deliver excellent and compassionate nursing and value our clinical teams as the real heart of the NHS. ### Main duties of the job

We are looking for an experienced community nurse, looking for the next step in their career, community nursing services are expanding in line with the 10 year plan to provide more care in the community, this is an exciting place to be at this time. We are seeking a motivated and passionate Senior Nurse to support the community teams to deliver person centred, research based care. You will have the skills and experience to manage your own caseload, which will be aligned to the GP practice and  residential homes within the area. You will be working with the Biggleswade 1 Team delivering person centred care to people in the place they call home. Excellent communication and time management skills, and sound clinical knowledge, are essential skills.  A flexible approach is required to accommodate the changing needs of the service along with the drive to influence service development. We are seeking someone with significant community experience to manage the caseload.

You must have a driving license and have access to a vehicle in order to carry out your duties.  You will be required to work a 7.5 hour shift pattern covering 7 days a week.

### Other

**Essential**

- Current driving licence
- Ability to travel across sites and across Bedfordshire to meet patient needs and attend meetings, etc

### Education

**Essential**

- Registered General Nurse
- Demonstrate evidence-based practice and up-to-date clinical skills
- Evidence of significant post registration nursing experience including community setting
- Post-registration qualification in a relevant clinical field
- Relevant teaching/mentorship qualification and evidence of teaching skills.
- Post-registration nursing experience, including community experience

**Desirable**

- Specialist Practitioner Qualification
- Nurse Prescriber

### Knowledge

**Essential**

- Literate in IT/Computer Skills
- Knowledge of the principles of Safeguarding of Vulnerable Adults and Children
- Knowledge of the principles of health promotion and prevention of illness
- Good oral communication skills based on fluency on the English language

**Desirable**

- Demonstrate an understanding of national government initiatives in health & social care & how they impact on local services
- - To utilise systmone data and other performance and activity data to influence practice and improve local service delivery.

- Evidence of and commitment to multi-professional working
- Understanding of the interface between primary and secondary care
- Comprehensive understanding of partnership working
- Ability to manage a caseload
- Be able to work in collaboration with patients, carers and the MDT

- Understanding of safeguarding and essentials of managing complex cases
- Be a proactive nurse with a strong desire to lead on the delivery of quality, holistic care
- Be a positive role model and mentor to support junior staff in their day to day clinical activities
