### Job overview

The post holder will be responsible and provide operational management and clinical leadership for the community cardiac and respiratory clinical team to deliver optimal care in line with the Care Closer to Home model and emerging Neighbourhood Health.

Use highly developed specialist knowledge to assess, plan, implement and evaluate individualised care for patients with heart failure and respiratory conditions. Ensuring patients are receiving correct evidence-based treatments in line with local policies and NICE guidelines.

The Clinical Manager will provide vision and professional clinical leadership to the Community Heart Failure Team and the Community Respiratory Team and will identify and support internal clinical development and education to ensure high quality person-centred care is maintained which will be dynamic, proactive ad responsive to health and social needs.

To ensure that services are responsive, innovative and operate to the highest possible clinical, performance and quality standards in line with latest guidance within Heart Failure and Respiratory care. Will be responsibility for keeping other team members up to date with any changes to the service delivery.

Promote multidisciplinary working with system partners, including Hospital at Home, UCRT, CATU, ICCTs, PCNs across West Essex.

To provide advice and guidance to other staff on complex cases.

### Main duties of the job

Manage and lead the multi-professional team in the delivery of safe and effective evidence-based interventions.

Provide a visible, accessible and professional presence for staff, service users and their families in relation to the Community Cardiac and Community Respiratory services

To deliver a service within the agreed financial envelope ensuring

Ensure continuity of service delivery by working collaboratively with key professionals.

Organise and co-chair in service team meetings working collaboratively with colleagues under the management of the Integrated Service Manager

Contribute to effective communication and liaison with system partners and with other services and professionals including statutory and voluntary agencies and community groups.

Act as a resource for advice and expertise to staff, users, carers and other agencies such as GPs and other primary care and ICB colleagues.

Support all direct reports to plan for 100% compliance with statutory and mandatory training for nursing staff on an annual basis

Ensure performance requirements are met through the production of required statistics and data.

Represent the Trust at system partnership meetings i.e. Expert Oversight Groups, ICS meetings.

Undertake projects or tasks outside the normal responsibilities when required.

### Detailed job description and main responsibilities

Generic Managerial Responsibilities both within an operational and strategic context

- Support all direct reports to plan for 100% compliance with appraisals and personal development plans for all nursing staff on an annual basis. Act as a member of the Community Health Services Leadership Team and contribute to the development of policy formulation, planning and resource allocation.
- Assist in the investigation of complaints and incidents under the direction of the Operational Service Manager, Deputy Director and Director
- Lead multidisciplinary / multiagency practice development and service improvement projects.
- Ensure the safeguarding agenda within community heart failure team and community respiratory team is implemented with effective systems throughout the service including effective liaison with other agencies as required
- Ensure that the Service User and Carers are actively involved in the planning and delivery of services in keeping with the Trusts engagement policies

Finance

- To authorise shifts using E Rostering.
- As part of the Directorate management team manage the delegated budgets and ensure the financial targets for the team is met
- Comply with financial processes, standing financial instructions and deadlines
- Ensure relevant financial information is presented to staff and meetings, as appropriate

Performance management

- To actively engage with the Neighbourhood Working Agenda, lead on new developments and inform planning of new services
- To actively engage and ensure integration with Primary Care Networks
- Develop effective working relationships with external stakeholders
- Deputise for the Operational Service Manager
- Ensure that regular audits and evaluations are undertaken to determine compliance with required CQC and professional standards.
- Ensure arrangements are in place to achieve all of the teams’ performance targets.

Human Resources

- Participate in the recruitment and selection of staff.
- Manage attendance and participate in disciplinary and grievance matters when necessary.
- Undertake staff performance reviews and appraisals with team members, identifying staff development needs and agreeing opportunities for these needs to be met.
- To manage appropriately all new starters in line with Trust policy and procedure.
- To assist in the management of sickness by ensuring all return to work interviews are completed and review meetings are held with the team manager

## Person Specification

### Knowledge

**Essential**

- Demonstrates up to date knowledge in Cardiac Management, Cardiac Rehabilitation Heart Failure and Respiratory Management
- Evidence of experience within operational management in a senior nurse role. Excellent leadership skills with the ability to enthuse motivate and involve individuals and teams.
- Knowledge and experience of leading change, demonstrating sustained improvement
- Understanding of Integrated Care Systems, Primary Care Networks and local health and care systems

### Education/Qualifications

**Essential**

- Experience within heart failure, cardiac rehab and respiratory community services
- non-medical prescribing qualification V300 or willingness to undertake

**Desirable**

- Experience in organisational development and change management
- Relevant postgraduate qualification in Cardiology, Respiratory Care, Long-Term Conditions, Heart Failure, COPD, Integrated Care, or Advanced Clinical Practice.
- Management/Leadership Qualification
