An exciting opportunity has arisen for a permanent, part time (30 hours per week) Community Assessment Officer within the Sensory Team in Salford.

The Sensory Team is part of the Community Learning Disability, Sensory and Transition Service, which is part of Adult Social Care.

This is a truly integrated service where you are based together with Social Workers, Community Assessment Officers, Vision Rehabilitation Specialists, Deafblind Assessor, Deafblind Communicator Guide, Community Nurses and Allied Health Professionals.

We are looking to recruit an enthusiastic and motivated individual who has a passion for working with adults with sensory needs, understanding their aspirations and supporting them to enhance their quality of life.

The person must be committed to supporting others, enabling effective integrated working, promoting independence through reablement approaches and Safeguarding adults in Salford.

You will be responsible for carrying out Sensory Need assessments and supporting sessions within the community alongside care management duties, requiring the highest levels of skills, knowledge and professional expertise.

You will assist the Team Manager and wider team in ensuring the delivery of effective, safe integrated services for adults with sensory needs.

This will be achieved through the promotion of high standards of practice in strengths based assessments and person centred support planning, whilst being creative to meet individual outcomes.

### Main duties of the job

The successful candidate will be a skilled individual able to work with adults with sensory needs who have varying goals and desired outcomes.

They should have a good understanding of the needs of adults with sensory needs and be able to work effectively with other professionals.

They will be person centred and have a good knowledge of relevant legislation such as The Care Act 2014 and The Mental Capacity Act 2005.

They will be able to balance risk whilst empowering people, promoting self-determination, strengths based working and reablement.

They will need to network and liaise with a wider range of professionals, colleagues, community services, people and their families and carers.

They will need to manage their workload, contribute to the development of the team, and be able to manage confrontation and complex relationships effectively.

Potential TUPE Transfer

Adult Social Care services in Salford are currently delivered by Northern Care Alliance NHS Foundation Trust and are expected to transfer to Salford City Council during 2026.

As this role forms part of those services, the successful candidate is likely to transfer to Salford City Council under the Transfer of Undertakings (Protection of Employment) Regulations 2006 (TUPE), subject to the transfer proceeding as planned. Any transfer would be managed through the appropriate formal process, including consultation and support for affected employees.

### Education

- A general education commensurate with the need to keep sensitive, concise and accurate written records, and ability to communicate information appropriately, clearly and concisely both verbally and in written form.

**Desirable**

- Qualification in British Sign Language.

- An understanding of the role of Community & Social Care and of the relevant legislation in the provision of personal social Care and Health.
- To have an understanding of the Mental Capacity Act and the requirement of best interest decisions should a person not have the capacity to make decisions regarding their care and support
- To have a good understanding of the local community, statutory and non - statutory services
- To have knowledge of and work within relevant organisational policies and procedures
- To support less experienced staff and mentor new staff and / or students from across Adult Health and Social Care Services
- To liaise with other appropriate Adult Health and Social Care professionals / teams as may be necessary during the course of the work
- Ability to identify the range of service user/carer needs, devising care and support plans and re-negotiating as service user/carer needs and circumstances change.
- Awareness of the effects and implications of illness and disability on people and their families/carers.
- Ability to develop and maintain positive working relationships
