Working within a well-established multidisciplinary team, you will deliver high-quality, patient-centred care and collaborate with colleagues across the organisation and specialist haemoglobinopathy network to ensure effective care coordination and positive patient outcomes. As a senior specialist nurse, you will act as a key resource for patients, families and healthcare professionals, providing specialist advice, education and support throughout the patient pathway. You will build strong therapeutic relationships, empowering patients to manage their condition and participate in decisions about their care. The role includes clinical leadership responsibilities, promoting evidence-based practice, supporting colleagues and contributing to service development, quality improvement and innovation. You will help shape the future delivery of haemoglobinopathy services while ensuring care remains responsive to the needs of patients and families. This is an excellent opportunity for an experienced nurse to further develop their specialist expertise and leadership skills while making a meaningful difference to the lives of people living with haemoglobinopathies. ### Main duties of the job In conjunction with the haemoglobinopathies teams across North Central London, the post holder will be responsible for the delivery and ongoing development of a high-quality specialist sickle cell and thalassemia service in the community. The post holder will provide clinical leadership and practice at an advanced level demonstrating a high level of autonomy and clinical decision making in the community. Implementation of a patient outreach health education group programme, and organisation of patient led education sessions.
