### Job overview

At Wye Valley NHS Trust the Colorectal/Stoma Care Nursing Team provide holistic support to patients and their families and carers on their Colorectal and oncology pathways. As part of the Multi-disciplinary team the successful post holder will be instrumental in helping to develop new pathways as part of an evolving Service in order to meet the needs of our patients both in the acute and community settings. We are involved in all aspects of prevention, diagnosis, treatment and management of patients with colorectal disorders.

### Main duties of the job

- Provide specialist Colorectal/Stoma care advice and support to patients, their families and carers and other healthcare providers throughout the patients care pathway. This will take place in both an in-patient and out-patient setting.
- Work as a core member of the Multidisciplinary Colorectal Team at the Wye Valley Trust contributing to meetings, audits and care pathways as required.
- Be an active participant in already established Nurse Led pathways and will be offered additional training in order to support this element of the role.
- Provide specialist nursing support to Colorectal clinics and act as a point of contact and resource for patients, their families and members of the wider MDT
- Participate in the education and training of patients, family and carers and other health care providers within the speciality.
- Make use of audit and research data to evaluate the service and ensure that best evidence based care is provided.
- Be actively involved in health promotion and prevention strategies and adhere to Wye Valley Trust policies and procedures.

### Experience

**Essential**

- 5 years post registration
- Advanced level of patient assessment skills
- Demonstrate an understanding of Colorectal pathways
- Experience of implementing change

### Education and Qualifications
