As part of the multi-disciplinary team the successful post holder will be instrumental in helping to develop a quality specialist Colorectal nursing service and new pathways as part of an evolving Service to meet the needs of our patients both in the acute and community settings. We are involved in all aspects of prevention, diagnosis, treatment and management of patients with colorectal disorders. ### Main duties of the job The post holder should be highly motivated and demonstrate excellent communication and interpersonal skills. They will be required to work autonomously and as part of a team and in order to cope with the demands of the role will need to be flexible and resilient and demonstrate sensitivity to others in sometimes distressing situations. The post holder will deliver high quality and clinically effective colorectal / stoma care nursing to the populous of Herefordshire and South Powys based at Hereford County Hospital. They will act as a key worker for each individual patients for all stages of the colorectal/stoma care pathway providing holistic support to patients and their families and carers. The post holder will be expected to promote education and information to raise awareness of colorectal / stoma care issues to health care professionals and the public. Incorporate research findings into clinical practice and contribute to the achievement of corporate objectives. ### Detailed job description and main responsibilities - Provide specialist Colorectal/Stoma care advice and support to patients, their families and carers and other healthcare providers throughout the patients care pathway.
