### Job overview

The post holder will be a key member of the Primary Care Department and hold the responsibility for leading the development of PTHB Clusters and assisting the Head of Primary Care in the development and delivery of long-term operational service and strategic plans.  Lead on oversight, assurance and delivery of a range of projects, programmes and local plans.   Ensure a robust and proactive approach to communicate and engage with all stakeholders internal and external to PTHB, including patient population.   Work closely with other departments and groups across Powys to ensure interdependencies are identified, considered and appropriately responded to.   Interpret and lead on implementation of relevant strategic and operational, national and organisational policies including the Community by Design programme.  The post holder will need to adapt and evolve to local, regional and national changes which impact the integration of Primary and Community Care.

### Main duties of the job

Lead the Cluster Development Team and ensure a high standard of professionalism, efficiency, and effectiveness in the delivery of the objectives of the Department and the Health Board; this will include resource planning, performance management, standard setting, team and individual development together with coaching and mentoring where appropriate.   Lead on the delivery of the Community by Design programme, ensuring our local models deliver expected national outcomes and include aligning with Clusters, wider Primary and Community Care integration, and report locally and nationally.

- Ability to travel between sites in a timely manner, including travel external to PTHB footprint

### Experience

**Essential**

- Significant experience of working within the planning regime and of dealing with other agencies in the statutory and non-statutory Health and Social Care sector
- Developing proposals into firm business cases for consideration by senior managers and funding bodies
- Working within a multidisciplinary internal NHS team and with wider partners at Senior Manager level
- Good track record of establishing positive relationships with partner agencies and professionals
- Effective financial planning, management and delivery within budgets
- Managing a team including setting objectives and performance monitoring
- Managing and resolving conflict and difficult situations
- Presenting to large audiences – both operational and strategic audiences
- Significant experience of scaling small projects into wider service delivery solutions

**Desirable**

- Experience of developing and commissioning capital plans
- Experience of senior management within the Cluster setting

### Qualifications and Knowledge

**Essential**

- Evidence of master's degree or experience of working to that level
- Post-graduate management or equivalent experience
- Evidence of continuing professional development
- High level of understanding of Health and/ or Social Care management environment in Wales, and roles and
- Highly specialist knowledge base that facilitates communication at high management level
- Excellent knowledge of the national strategic and policy context, and planning and pertinent partnership structures
- Knowledge and experience of working with pooled budgets and other areas where resources can be shared
- Understanding the key challenges and opportunities within PTHB

**Desirable**

- Expert knowledge of all Cluster related programmes
- Experience of working within a Cluster Programme
- Expert knowledge of the Community by Design programme
