\<p\>\<strong\>Clinical Triage Practitioner\</strong\>\</p\>\<p\>An exciting opportunity has arisen for a Clinical Triage Practitioner to \<span class="single_desc_int_gb_word"\>support the delivery of Children\\'s Specialist Services across Bath & North East Somerset, Swindon and Wiltshire\</span\>.\</p\>\<p\>The post holder will undertake clinical triage of referrals, including urgent referrals, ensuring timely assessment, \<span class="single_desc_int_gb_word"\>\<span class="single_desc_int_gb_word"\>decision-making and communication of outcomes\</span\>. Working as part of a multidisciplinary team, the successful candidate will \<span class="single_desc_int_gb_word"\>support complex referral discussions, provide clinical advice and contribute to effective care coordination across services\</span\>.\</span\>\</p\>\<p\>The role requires close partnership working with healthcare professionals, education services, voluntary sector organisations and families to ensure children and young people receive appropriate, safe and effective \<span class="single_desc_int_gb_word"\>support through a coordinated and integrated care pathway\</span\>.\</p\>\<p\>\<strong\>\<strong\>\<strong\>Location:\</strong\> \</strong\>\</strong\>To be agreed within Bath & North East Somerset, Swindon or Wiltshire, with hybrid home working
