An exciting opportunity has arisen for an experienced and motivated leader to join our Community Nursing Service as a Team Lead.

As part of our evolving Neighbourhood Team model, you will play a pivotal role in leading multidisciplinary community teams to deliver high-quality, safe and effective care to patients in their own homes and local communities. You will provide visible leadership, operational oversight and professional support to a diverse workforce, ensuring services are responsive, person-centred and aligned to organisational priorities.

### Main duties of the job

Main responsibilities include;

- Providing visible leadership and support to staff, creating a positive team culture that promotes wellbeing, engagement and continuous improvement.
- Managing workforce performance, recruitment, attendance, supervision and professional development.
- Overseeing the day-to-day delivery of services, ensuring resources are effectively deployed to meet patient and service needs.
- Working collaboratively with clinical, operational and multidisciplinary colleagues to coordinate care and improve outcomes for patients.
- Monitoring quality, performance and patient experience, taking action to address risks, concerns and opportunities for improvement.
- Leading and supporting service developments, quality improvement initiatives and organisational change programmes.
- Ensuring compliance with professional, organisational and regulatory standards, maintaining a strong focus on patient safety and governance.
