### Job summary

We have the opportunity for a suitably qualified RN to join our Frailty Teams. As part of this exciting, varied role you will be working with in the, Frailty Assessment Unit.

Main duties:

Conduct initial triage and assess patients who present to the Frailty Assessment Unit, Hospital at Home and Acute Frailty Team.

Perform a comprehensive assessment of the patient's condition, including physical exams, history taking, and diagnostic tests.

Prioritise and provide appropriate care, ensuring patients receive timely interventions.

Administer medications, treatments, and IV therapy as needed.

Rapid Diagnosis and Treatment:

Collaborate with doctors and other healthcare professionals to quickly diagnose and treat patients.

Perform or assist in minor procedures, such as wound care, suturing, and administering injections.

Ensure patients are stabilised and treated efficiently.

They will continuously develop and enhance knowledge and skills that will directly influence consistent

Communicate effectively with all the multi- disciplinary teams both internal and external to the

### Job responsibilities
