### Job summary

To cover maternity leave we
have the opportunity for a suitably qualified RN to join our Frailty Teams on a
band 6 Secondment role. As part of this exciting, varied role you will be
working with the Acute Frailty Team, Frailty Assessment Unit and Hospital at
Home services.

Main duties:

Conduct initial triage and
assess patients who present to the Frailty Assessment Unit, Hospital at Home
and Acute Frailty Team.

Perform a comprehensive
assessment of the patient's condition, including physical exams, history
taking, and diagnostic tests.

Prioritise and provide
appropriate care, ensuring patients receive timely interventions.

Administer medications,
treatments, and IV therapy as needed.

Rapid Diagnosis and
Treatment:

Collaborate with doctors and
other healthcare professionals to quickly diagnose and treat patients.

Perform or assist in minor
procedures, such as wound care, suturing, and administering injections.

Ensure patients are stabilised
and treated efficiently.

expertise and organisation working between the 3 Frailty Teams.

They will continuously develop and enhance knowledge and
skills that will directly influence consistent standards of clinical practice and ensure the highest
possible standards of evidence-based care are achieved in order to promote patient focused care.

They will promote patient centred high quality
evidence-based care to patients, ensuring the patient is central to all aspects of treatment and care.

Communicate effectively with all the multi- disciplinary
teams both internal and external to the organisation.

Provide clinical leadership to a group of staff within the
ward and throughout the care group.

Patient Assessment and Care:

Hospital at Home

Perform a comprehensive assessment of the patient's
condition, including physical exams, history taking, and diagnostic tests.

Prioritize and provide appropriate care, ensuring patients
receive timely interventions.

Administer medications, treatments, and IV
therapy as needed.

We provide hospital services from two acute sites - Darlington Memorial Hospital and University Hospital of North Durham. We have a centre for planned care in Bishop Auckland and provide care from community hospitals in Chester-le-Street, Shotley Bridge, Barnard Castle, Sedgefield and Weardale as well as over 80 other community based settings and providing care in patients' homes.
