We are looking for a skilled, enthusiastic, and experienced Clinical Pharmacist to join the Nottingham Division of Modality Partnership on a full-time basis for 37.5 hours, based at Wellspring Surgery. In this role, you will be instrumental in delivering high-quality, patient-centred pharmaceutical care, optimising medicines safely and effectively, and driving innovation in Primary Care.

### Main duties of the job

This is a dynamic, patient-facing Clinical Pharmacist role, focused on delivering evidence-based pharmaceutical care in Primary Care. You will use your expert knowledge of medicines to support patients with specific disease areas and long-term conditions.

Key responsibilities include:

- Clinically assessing, diagnosing, and prescribing (where qualified)
- Optimising medicines and improving prescribing safety
- Collaborating with a multidisciplinary team to enhance patient outcomes
- Supporting medication reviews and structured medicines optimisation
- Contributing to service development and innovative working practices
- Working with Divisional Boards and the National Modality Business Support Team to deliver strategic priorities

### Qualifications

- Masters degree in pharmacy (MPharm) or equivalent recognised pharmacy qualification.
- Registered as a Pharmacist with the General Pharmaceutical Council (GPhC), with no restrictions on practice.
- Independent Prescriber qualification
- Evidence of continuing professional development (CPD) relevant to clinical pharmacy practice.

**Desirable**

- Postgraduate clinical pharmacy qualification (e.g. Clinical Diploma or equivalent).
- Membership or Fellowship of the Royal Pharmaceutical Society (RPS).
- Primary Care Pharmacy Education Pathway (PCPEP) qualification or evidence of working towards completion.
- Qualification or training in long-term condition management, medicines optimisation or prescribing.
