Dementia Intensive Support Team West Suffolk

Non Medical Prescriber

Are you an experienced nurse looking to join a forward-thinking team? Do you want to support the local community in transformation by being part of the ongoing development of a service?

Our Dementia Intensive Support Team in West Suffolk (DIST) are seeking a Band 7 Prescriber to join their collaborative team.

DIST work with people across the locality with a cognitive impairment and/or dementia with a presentation of behavioural and psychological symptoms of dementia (BPSD). We screen, triage, and assess all referrals into the service, with a focus on positive outcomes for both service users and their carers.

It is an exciting time to join as we’re embracing a new venture with Trust transformation and development regarding referrals,  being led by motivating and supportive leadership. We have been further developing the service over the last year with the intention of meeting the changing and complex needs of the people we serve.

We’re based at G block, Bury St Edmunds, and work across the Waveney patch West Suffolk, you’ll have a varied workload and will be supported by the wider MDT.

We are looking for professionals who are caring, supportive, proactive and reflective in their practice. If this sounds like you – we’d love to hear from you!

### Main duties of the job

Complete holistic mental health assessments with Medication review and prescribing.

- Undertake the assessment, planning and delivery of treatment with minimal supervision, ensuring continuity of practice and working within a multi-disciplinary and multi-agency environment.
- Communicate decisions about mental health care services, referrals and future support arrangements to people with a mental health care need.
- Be responsible for accurate recording of all patients support, including assessments, reviews, specific prescriptions and day to day contact and discharges.

The successful candidate will join a service with a passion for affecting outcomes for people with dementia, focused on learning, developing, improving, and supporting each other as well as our service users.

General responsibilities include:

- Having a good understanding of community mental health and national dementia agendas.

- Development of robust relationships with partner agencies and services.

- You will be expected to be able to work autonomously and manage your time well.

-You must be able to be able to travel to various locations across West Suffolk

- Confident manner. Approachable, tactful and diplomatic.
- Able to demonstrate sound clinical expertise and knowledge of mental health disorders and their treatments.

### Experience

**Essential**

- Extensive experience at which has required supervision and mentorship of others.
- Extensive experience of practice within clinical area with proven track record in delivering high quality care.

**Desirable**

- Teaching or training experience or qualification.

### Qualifications

**Essential**

- RMN or equivalent professional qualification.
- Non-Medical Independent Prescriber.
