We are delighted to offer this exciting opportunity, for an experienced nurse to join our Specialist Palliative Care Team (SPCT), currently consisting of Hospital and Community Palliative Care Nurse Specialists, Hospice at Home Nurses, Consultants in Palliative Medicine, Health Care Support Workers and Admin and Clerical Team.

The SPCT provides specialist support, care and advice to patients and their families living with life limiting illness, in Anglesey and Gwynedd, across primary and secondary care.

The post holder will contribute to the delivery of a high quality, accessible palliative care service, providing specialist support, care and advice to patients and their families living with life limiting disease. The post holder will be expected to work week days and regular weekends, during the hours of 9 - 5pm.

## Person Specification

### essential

**Essential**

- 1st Level Registered General Nurse. Post Registration qualification in relevant area or knowledge gained through equivalent experience. Relevant First Degree or working towards, or knowledge gained through equivalent experience.
- Significant post registration experience, in relevant specialty Experience of working effectively as part of the multidisciplinary team.
- Ability to independently make clinical decisions. Ability to work both within a team and autonomously. Effective time management and organisational skills Advanced communication skills. Ability to influence change through application of advanced clinical knowledge and skills.
- Knowledge of: Advances and current issues in palliative care. Health care policy and strategy relevant to palliative care. Good understanding of audit and research and its application to / uses in practice. Clinical and information governance. The Scope of Professional Practice / Code of Conduct.
- Ability to work flexibly to meet the demands of the service. Ability to motivate and lead others. Ability to manage opposition and overcome obstacles in a positive and non-threatening manner.
- Ability to make own transport arrangements for home visits / to travel between sites.

**Desirable**

- Non Medical Independent Prescriber Teaching qualification or knowledge gained through equivalent experience
- Experience of participation in research and audit. Good IT skills (word, email, PowerPoint)
- Fluency in the Welsh Language
