This is a excellent opportunity to join the regional specialist palliative care to support CYP and their families with life limiting, palliative and end of life care needs across the region.

The post holder will plan, deliver and evaluate care, following the co-production of care plans to ensure the needs and wishes of CYP and their families are being met.

The post holder will have the knowledge and skills to support nursing colleagues and other health professionals in the delivery of end of life care, either over the telephone or in person.

### Main duties of the job

Reporting into the clinical nurse manager, some of your responsibilities will include:

- Assess, plan and review specialist complex care for children and young people, working within agreed care pathways and alongside the multidisciplinary team
- Deliver care through nurse-led clinics, telephone support and home visits
- Monitor patients' conditions and respond appropriately to changes in clinical need
- Provide specialist advice, education and training to patients, families, carers and healthcare professionals
- Coordinate care with multidisciplinary teams and external agencies to ensure continuity and seamless transitions
- Provide out-of-hours telephone support for children and families receiving end-of-life care.
- Undertake out-of-hours visits to support children, young people and families with end-of-life care needs
- Maintain accurate clinical records and contribute to service improvement and best practice initiatives.

## Person Specification

### Knowledge

**Essential**

- Ability to dynamically risk assess clinical situations and respond appropriately.
- Ability to monitor and respond to changes in patient conditions effectively.
- Effective communication skills to provide guidance and education to patients.

### Experience

**Essential**

- Experience of providing clinical advice in relation to the specialist area.
- Experience of planning provision of care for patients
- Mentoring experience with nursing students and training/development teaching of junior colleagues

### Qualifications

**Essential**

- RN degree or diploma
- Current NMC registration
- Relevant post registration qualification as required for speciality
- 998 or mentorship / supervisor training

**Desirable**

- Clinical skills and pathophysiology module (or willingness to undertake depending on role within speciality
- Non-medical prescribing

### Personal Attributes

**Essential**

- Ability to respect the cultural, social, and religious diversity within society.
- Ability to manage complex and challenging situations with empathy.
