Based at UCLH and across NCL, you’ll be part of a small, dedicated homeless intermediate care team working across our NCL acute hospital trusts, P2 beds and community to support patients experiencing homelessness and social exclusion. You will play a key role in helping patients navigate their health and discharge journey from admission to discharge and in community follow up working closely with clinical teams, local authorities, and community services to ensure safe, appropriate, and person-centred transitions from hospital to community care. You will also line manage a team of clinicians and non-clinicians in supporting our patients, focusing on staff improvement and wellbeing as well as continuous service improvement for our patients.
