It is an opportunity to work within a specialist team who are part of an international and national network of CF teams sharing information, knowledge and research to improve the lives of CF patients. The 0.6 WTE role will allow you to develop clinical expertise in managing the chronic health needs of CF patients as well as the complex psychosocial support a family requires. ### Main duties of the job Good clinical assessment skills are required to support your decision making and escalation of care as well as an ability to teach families.  We will support your development with a tailored educational pathway. This post will contribute on the coordination and management of the transition to adult healthcare pathway for children and young people with CF from the age of 12yrs, and provide clinical care, respiratory assessment, education, and psychosocial support in the outreach setting visiting CF patients at home, school and in their local hospitals.
