Are you an enthusiastic and motivated dietitian with expert skills in Diabetes Dietetics? Are you passionate about helping individuals with Type 2 Diabetes achieve remission via evidenced based dietary and lifestyle approaches?

An exciting opportunity has arisen to join the well-established dynamic diabetes dietetic team at Hywel Dda which holds a high profile both within the wider therapies team and across the Health Board. We fully support continuing professional development, appraisal and supervision and actively enable this with protected sessions in individualised job plans.

This is a key role for a lead in Type 2 Remission: Diabetes Dietetics, involving both clinical and leadership responsibilities. This role offers a unique opportunity for someone with a passion to improve health outcomes across Hywel Dda and a commitment to develop and learn. ### Main duties of the job

You will:

- deliver expert level clinical Diabetes Dietetics, managing a complex caseload of patients with Diabetes focusing on those individuals looking to achieve Type 2 diabetes remission.
- Manage a diabetes caseload, informing multi-disciplinary care (inc. Primary care and stakeholders) and provide expert support and clinical supervision for dietitians / support workers / practice based learners/ administrators Health Board wide.
- Clinically lead the development, implementation, delivery and evaluation of best practice in relation to adult Type 2 Diabetes remission dietetics, Health Board wide.
- Work as an integral member of the adult diabetes multi-professional leadership team, driving excellent nutritional care through evidence based practice.
- Ensure seamless service planning and delivery from promotion of service, recruitment of service users and evaluation of intervention.
- Clinically lead the development and delivery of education and training in relation to diabetes working across Multi profession platforms and multi-disciplinary teams (MDT) to support optimal effective diabetes care.
- Compassionately and effectively line manage members of the diabetes dietetic team.

- Broad well established Dietetic practice experience within NHS or comparable healthcare settings inc. primary care
- Extensive clinical dietetic delivery in Type 2 Diabetes Dietetics
- Experience of Clinical leadership
- Extensive multi-professional working within the NHS or equivalent healthcare setting inc. primary care
- Delivering clinical supervision
- Delegation to qualified and non-qualified team members.
- Significant experience of leading multi-professional and multidisciplinary team working
- Leading project / quality improvement / development work, with demonstrable outcomes alongside the ability to evaluate clinical and patient experience outcomes to show value.
- Active engagement with student / practice based learners training.
- Experience of interpreting evidence into clinical practice.

**Desirable**

- Experience of project management
- Additional clinical or health care related experience
- Experience in research/leading multiagency projects
- Leading service improvement /quality improvement
- Active involvement in national work stream, professional network or specialist group.

### Qualifications and Knowledge

**Essential**

- HCPC registration with a Degree in Dietetics
- Accredited post graduate study relevant to Diabetes to master's level or equivalent
- Completed practice based educator / clinical supervision training
- Advanced clinical knowledge within the specialist field of Type 2 Diabetes / Diabetes Remission and dietetics acquired through specialist training or equivalent clinical experience e.g. Counterweight Plus Programme
- Knowledge of, and ability to apply evidence-based methodology to support improvement, and service delivery.
- Expert knowledge of clinical guidance and policy relevant to the specialist area

**Desirable**

- Completed Formal Motivational interviewing training
- Supplementary prescriber status
- Trained in the delivery of recognised Diabetes Remission programmes such as Counterweight Plus
- Other professional, technical or academic qualifications which relates to Type 2 Diabetes e.g. XPERT / XPERT Insulin / Desmond
