### Job summary

The purpose of this role is to provide an efficient, effective, accurate, reliable and responsive clinical coding service for Horder Healthcare (HH).

To provide data on Horder Healthcares clinical activities, based on clinical coding, by abstracting information from a variety of documents and translating into diagnostic (ICD10) and procedure coding (relevant version of OPCS).

To work within Horder Healthcares clinical coding team, collaboratively leading on all coding matters, providing advice and guidance as required, and ensuring the Centre remains abreast of new developments and best practice.

### Main duties of the job

Abstract and interpret complex clinical information from case notes, discharge summaries and other records, assigning accurate ICD-10 and OPCS-4 codes in line with national standards and clinical coding conventions.

Ensure timely and accurate coding of diagnoses, co-morbidities and procedures, maintaining patient management systems and meeting monthly coding deadlines. Maintain up-to-date coding knowledge through continuous professional development and adherence to NHS Digital standards.

Support the maintenance and development of coding systems, processes and policies, including updates to ICD and OPCS classifications. Contribute to internal and external audits and promote high-quality clinical data through accurate coding practices.

Build effective relationships with consultants, clinical teams, nursing staff, administrators and finance colleagues to improve documentation quality, coding accuracy and HRG outcomes. Communicate complex coding guidance clearly through written, verbal and electronic methods.

Deliver training and advice to staff on the importance of accurate clinical documentation and coding. Identify opportunities to improve coding processes, data quality, business continuity and revenue generation. Work collaboratively with finance teams to resolve HRG discrepancies and NHS provider queries, while supporting projects that enhance coding performance and organisational effectiveness.
