An exciting opportunity has arisen for a forward thinking, enthusiastic, highly motivated and experienced Clinical care co-ordinator to lead a team of Community Health Support Workers (CHSW’s) in an innovative Reablement Service - Carelink. The service delivers high quality enabling support to Camden service users who have had a short term stay in hospital or have recently become unwell at home. The aim of the service in these cases, is to support the early discharge and prevent the hospital admission. The service also supports with palliative end of life care and community Phlebotomy for housebound patients.

### Main duties of the job

Service users on these pathways, because of their physical disability or illness are unable at present (in varying degrees), to complete their activities of daily living independently. The CHSW’s will work with these service users with the aim to progress the individual back to their base levels of independence. The role is split in to two main areas, Senior Reablement support worker and administration co-ordinator. As a senior support worker, the co-ordinators will supervise a team of support workers which includes training staff , assessment of staff competencies, quality monitoring, completing first assessments, weekly review of current care packages and to be rostered to be the main lead in charge of shifts.

### Detailed job description and main responsibilities

- Supervising duties
- To have supervisory responsibility for CHSWs in their work, inclusive of carrying out regular supervision. The post holder will be required to manage CHSW’s and their workload within a geographical arear and monitor quality and performance of support workers and where necessary initiate informal HR policies and process. This may involve return to work meetings after staff absence. Performance monitoring would include but not limited to: Adhesion to local and trust policies and guidelines, that apply to the service and ensure that delivery of support meets the service standard Effective verbal communication with clients, families, colleagues and professionals working alongside Carelink Supporting enabling theory at all times for appropriate client Clear and accurate client records both written and electronic as required according to local policies Appropriate use of client filling system To coordinate the use of resources where required, eg Oyster cards, PPE.
- To supervise the shift and be the link person for the Community Health Support Workers (CHSW) by providing advice and support as and when needed
- When supervising the shift - to oversee the rostering of shifts and ensure scheduled visits are delivered in order to meet the needs of the clients
- To take an active role in the appointed community locality when not undertaking the admin function by supervising shifts, conducting reviews, quality checks, competency assessments and where required support and train staff
- To undertake visits to clients homes independently as the locality co-ordinator and ensure first visits assessments, risk assessments and care plan reviews are carried out in a timely manner
- To take daily handover of client’s progress from CHSW’s and where appropriate report back to the deputy manager/clinical co-ordinator and other staff members in the team or any change in client’s condition or environment that may present health and safety and / or clinical issues for clients or other professionals
- To liaise with outside agencies such as Treating therapist, Adult Social Care, Adult Community Nursing and Specialist Palliative Care Team and hospital Accident and Emergency and Acute Medical Unit wards as required
- To monitor Carelink staff and ensure a high quality professional service is maintained i.e. adherence of uniform policy, health and safety, Infection control, information governance and recording client progress on SystmOne
- To lead on orientation and induction of new members of staff, including assessing staff competencies through a set framework as guided by the Carelink clinical co-ordinator
- To understand and ensure the potholder and the team adheres to local and trust policies and guidelines that apply to the service and ensure that delivery of support meets the service standard
- To carryout audits and client feedback on the service by conducting Carelink quality assurance questionnaires and friends and family test.
- To be the named key worker and establish, maintain effective communication with the client, their family, referring teams

- To have the ability and experience in applying physical skills and effort in the care of clients with complex disabilities in their own homes

### Experience

**Essential**

- Significant proven administrative experience
- Significant proven experience in supporting older people who are physically frail inc. providing personal care in clients homes
- Demonstrate experience of assessing, planning and implementing individualised program of care
- Experience of direct line management of non-clinical staff
- Managing staff performance and sickness management of staff
- Experience of dealing with non-routine & complex problems, chasing patient progress, resource & staff allocation
- Prove experience of working under pressure and managing a demanding workload
- Experience in carrying out initial assessments, Quality Assurance Reviews and care plan reviews
- Working in an NHS or Social Care setting

### Qualifications

**Essential**

- Educated to at least A level/NVQ level 3 in Health and Social Care

### Skills/Knowledge

**Essential**

- Advanced skills in Microsoft Office
- Excellent range of communication & prioritisation skills and good interpersonal skills
- Excellent planning and organisational skills
- Effective problem solving and decision making skills
- Knowledge of patient care needs according to care plan
