The City and Hackney Place Based Partnership is seeking a Director to lead its development and drive delivery of its strategic objectives including furthering the development of our neighbourhoods model

This is an exciting role charged with leading local health and care system partners into an accelerated phase of care transformation, integration, and service improvement for the residents of the City of London and Hackney.

The City and Hackney Place-based partnership is a collaborative that comprises the City of London Corporation, the London Borough of Hackney, Homerton Healthcare FT, East London NHS FT, City and Hackney Integrated Primary Care, representation from the C&H Charity and Voluntary Sector Leadership Group, along with lay and clinical leaders, which takes responsibility for:

- Working with local residents and communities to improve their health and wellbeing,
- addressing longstanding inequalities, and
- providing high quality, integrated and sustainable health and care

### Main duties of the job

- The Partnership Director will be responsible for supporting the Place-based partnership to deliver a focused programme of work to improve the health and wellbeing outcomes of people living in City and Hackney. Much of this will be delivered through a neighbourhoods approach.
- They will also support the ongoing development of the partnership in readiness for new contracting forms that can further enable delivery of our local vision.
- The City and Hackney Partnership Director will be employed by Homerton Healthcare but will work on behalf of the local partnership and will be accountable to the City and Hackney Partnership Board.

There is a long legacy of partnership working across City and Hackney which has evolved through maturing relationships and an increasingly aligned approach to population health and health inequalities across partner organisations. A central tenet of this approach has been recognising the opportunity from neighbourhood working to enable a left shit and deliver more integrated and preventative care. We are in the process of refreshing our partnership governance and ways of working in order to enable delivery of our ambitions for neighbourhoods and to support readiness for new contracting and system forms.
