An exciting opportunity has arisen for portfolio GPs wanting to work in our supported, Community Intermediate Care Centre (CICC).

CICC is a three-ward 71-bedded step-up/step-down Rehabilitation (Intermediate Care) Centre where you will, in effect, be the patient’s GP for the duration of their stay with the aim of maximising their wellbeing and supporting their rehab and discharge.

The successful applicant will form part of the multi-disciplinary team caring for patients within CICC, working closely with the Hospital and Community services to support the safe step-up and step-down of patients from community and hospital settings into the Unit.

### Main duties of the job

The successful candidate will be required to collaborate with our team of multi-disciplinary staff to facilitate the safe and timely admission, rehabilitation and discharge of patients in the Community Intermediate Care Centre. They will provide patient centred medical oversight and support for the assessment, treatment and management of patients on the Unit, working closely with the wider multi-disciplinary team to ensure comprehensive care.

They will maintain accurate electronic medical records, respond promptly to any changes in patients’ clinical conditions, adhering to the Unit’s escalation policy to ensure timely interventions and patient safety, making appropriate referrals to secondary care or other agencies when appropriate. Where needed, they will utilise lab diagnostics, including Microbiology, Haematology and Biochemistry.

They will be expected to foster a collaborative and supportive environment within the team, contributing to the overall success of patient care delivery and the discharging of patients back to their own home setting.

- Evidence of continuing professional development
