We are seeking an enthusiastic and experienced Children's Complex Case Manager to join our Children's Continuing Care Team. This is an exciting opportunity to play a pivotal role in ensuring that children and young people with complex health needs receive safe, effective and personalised continuing care services. As part of a specialist team, you will work closely with children, young people, families and partner organisations to coordinate high-quality care and support positive outcomes.
