We are seeking an enthusiastic and experienced Children's Complex Case Manager to join our Children's Continuing Care Team. This is an exciting opportunity to play a pivotal role in ensuring that children and young people with complex health needs receive safe, effective and personalised continuing care services.

As part of a specialist team, you will work closely with children, young people, families and partner organisations to coordinate high-quality care and support positive outcomes. This role offers the opportunity to utilise and further develop your expertise in continuing care, complex case management and multidisciplinary working across North West London.

### Main duties of the job

The Children's Complex Case Manager is the lead nurse assessor for children's and young people's continuing care and will be responsible for:

- Leading comprehensive continuing care assessments, reviews and reassessments for children and young people with complex health needs.
- Providing expert case management to ensure care is safe, effective, personalised and responsive to changing needs.
- Working closely with families, carers, advocates and service providers to coordinate and oversee high-quality packages of care.
- Sourcing and developing bespoke care solutions that meet individual needs and promote the best possible outcomes.
- Collaborating effectively with health, social care, education and provider organisations across North West London.
- Chairing and contributing to complex multidisciplinary meetings to support robust decision-making and care planning.
- Supporting the delivery of excellent clinical outcomes and ensuring services are provided in line with national Continuing Care guidance and local policies.

## Person Specification

### Experience

**Essential**

- Experience working as a nurse assessor or equivalent for continuing care /continuing healthcare (CHC)
- Experience of working with clinicians and/or within clinical settings to deliver excellent health outcomes for children and young people in care settings.
- Experience or a strong understanding of the policies, procedures and practices employed by clinicians and social workers to assess and review eligible clients in receipt of continuing care, children’s social care and SEND
- Experience of working in the NHS and/or local authority at a relevant level.
- Demonstrated experience of delivering best possible clinical outcomes for children and young people eligible for care services
- Significant experience of effective work with children and young people patients/service users, families and carers to deliver the best possible clinical outcomes
- Experience of chairing complex meetings across multidisciplinary teams
- Experience of successfully managing disputes between organisations or with clients and their families, including through recourse to legal avenues of redress where necessary.
- Ability to provide and receive highly complex, sensitive and contentious information, to negotiate with senior stakeholders on challenging and controversial issues, and to present complex and sensitive information to large and/or influential groups.

### Skills & Knowledge

- Excellent knowledge of continuing care (CC) guidance, policies and decision making.

### Education/Qualification

**Essential**

- First degree or equivalent professional qualification or management experience or knowledge acquired through experience or training.
- Registered children and young people’s nurse on the Nursing and Midwifery Council (NMC) register
- A Degree supplemented by diploma level specialist training, experience, short courses Registered children and young people’s nurse on the Nursing and Midwifery Council (NMC) register.
