We have an exciting opportunity for a motivated band 5 paediatric nurse to join our team as a band 5 Childrens Community Staff Nurse. The team provide nursing care to children and young people across County Durham and Darlington with a range of nursing needs. The service provides community care 7 days a week 08.00 to 20.00 and we have 2 office bases in Darlington Memorial Hospital and Chester-le Street Hospital. The successful candidate would be predominantly based in Darlington, however there would be an expectation that the post holder may need to work from either base to meet the needs of the service. This post is advertised for 36 hours per week, however consideration would be given to part-time or full time, if this meets the needs of the service. ### Main duties of the job You will work as part of a team of Children's Community Nursing Sisters, Staff nurses and Healthcare Assistants. The successful will provide a high standard of nursing care to children and their families in a variety of community settings. This will involve carrying out visits to family homes, schools, nurseries and short break provisions to ensure the child's nursing needs are met. You will work autonomously as a lone worker whilst on visits in the community, however there will always be a senior point of contact whilst on shift. Training will be provided to ensure that you have the necessary skills to deliver evidence based care and you will be supported to develop your own skills. As a band 5 Childrens Community Nurse we are looking for a dynamic, innovative individual who is passionate about delivering the highest level of patient care. In order for these children to access education and short break provisions you will participate in delivering education to carers. Children and Young people are often discharged home from the acute setting to continue their care at home for a period of time. You will therefore be expected to contribute to coordinating a safe discharge home and administer IV medications and carry out other nursing care to meet the child's needs in the community. As a band 5 Childrens Community Nurse you will not hold a caseload, however you will be assigned a caseload as an associate nurse, alongside the band 6 caseload holder. On occasion you will be expected to coordinate the care being delivered by the team on allocated shifts. This will require an understanding of the wider patient list, the role and geographical area covered by the service. Due to the nature of the role you will be required to travel across County Durham and Darlington; you must have your own vehicle in order to travel as part of the role.
