### Job overview

The role of a nurse for Children Looked After (CLA) requires personal and professional autonomy; the ability to make clinical judgements, assess need, risk, advocate for, and support the implementation of care planning and safeguarding CLA.

You will assess and identify health needs of CLA , creating  health reports and care plans, supporting professionals to ensure the plan is adhered to.

By working within a multi-agency context to identify health needs and promote health and well-being, you are required to provide direct/ outreach support . You will work in collaboration with colleagues within LGT and externally, as well as social care, education and youth justice system. You will be required to attend strategy meetings to support the safety and wellbeing of CLA.

You will be part of a dedicated team as a point of contact, delivering health advice to the professional network work in relation to the management of the physical, mental and emotional health and wellbeing of CLA.

### Main duties of the job

Undertake statutory health assessments, and associated activities for CLA, including those with severe trauma and adverse childhood experiences, disability or complex medical needs, special educational needs, attachment disorders, and unaccompanied asylum-seeking children (UASC).

Formulate SMART meaningful individual healthcare plans to coordinate health care and monitor implementation.

Be a point of contact for consultation, guidance and health support to other partner agencies and stakeholders involved in the care planning and/or delivery of care regarding the health management offer CLA.

Interpret and communicate on a broad range of health information in a social, education and contextual safeguarding context, across multi-agency stakeholders.

Be a key agency member of multi-agency meetings including highly confidential strategy meetings, professional meetings or child protection case conferences, providing reports and expertise on issues relating to CLA.

Support the Named Nurse and Specialist Nurses with service development/quality improvements.

Support with the delivery of training session

The post holder will be part of a team who are a dedicated point of contact, delivering health and wellbeing advice to all members of the professional network and corporate parent network in relation to the management of the physical, mental and emotional health and wellbeing of CLA.

The post holder will work closely with internal colleagues such as the Named Nurse for CLA & Care Leavers, Specialist Nurse for CLA, Specialist Nurse for Care Leavers 18-25yrs, Designated Doctor for Looked After, Medical Advisor for Adoption and Fostering as well as Local Authority partners in order to promote effective partnership working and achievement of statutory requirements.

## Person Specification

### Experience

**Essential**

- Significant experience as a community nurse working within School Nursing, Community Children's Nursing, Health Visiting or safeguarding children & young people – showing a strong understanding of public health issues and health inequalities relating to children and young people
- Experience of undertaking complex and holistic health assessments and creating care plans
- Sound knowledge of current issues and legislation relating to Children Looked After
- Understanding of diversity and commitment to equal opportunities and how it relates to Children Looked After
- Experience of safeguarding and working with vulnerable families
- Experience of interagency working

**Desirable**

- Experience of working within the Children Looked After arena
- Strong working knowledge of the frameworks, guidelines and principles underpinning the delivery of services to Children Looked After and Care Leavers
- Sexual & Reproductive Health experience
- Sound knowledge of trauma informed care delivery

### Knowledge and Skills

**Essential**

- Able to work autonomously and be skilled in professional decision making.
- Ability to assess needs and to develop SMART care plans to meet these
- Excellent computer skills -able to use word excel and power point
- Experience of delivery of evidence-based health promotion
- Excellent communication and organisational skills
- Good literacy and numeracy skills and the ability to produce reports
- Excellent interpersonal skills
- Able to work within a multi-agency arena Provide advice and guidance with tact and sensitivity.
- Ability to adapt to change within the working environment
- Car driver with clean UK Driving license & ability to travel to deliver services for children placed out of the borough

**Desirable**

- Strong working knowledge of the frameworks, guidelines and principles underpinning the delivery of services to Children Looked After and Care Leavers

### Qualifications and Training

**Essential**

- Registered Nurse (RCN, RGN, RMN)
- Trained to minimum of Level 3 in Safeguarding Children & Young People

**Desirable**

- Registered Children’s Nurse
- Specialist Community Public Health Nurse (HV/SN) qualification
