Using behaviour change techniques such as motivational interviewing, goal setting and confidence building, you will provide tailored support to improve health outcomes and reduce inequalities.\\You will work within family hubs, healthy child clinics, schools and community settings, delivering one-to-one and group interventions to families with children aged 019 years, and up to 25 years for young people with Special Educational Needs and Disabilities (SEND).\\This role also includes supporting families identified through the National Child Measurement Programme (NCMP) and signposting them to relevant serv ### Main duties of the job About the Company We change lives by transforming health and care.
