The All Age Continuing Care leads with the assessment, case management and oversight of the NHS National Framework for Continuing Healthcare (CHC) and Funded Nursing Care (FNC) across Kent and Medway. Assessing, developing and overseeing personalised care package for those with complex or long-term conditions both in their own homes and in care homes. Our teams work closely with the Local Authority, Primary Care Networks and system partners as part of a multidisciplinary system. ### Main duties of the job - Assessing people both in care homes and their own homes in accordance with the National Framework - Assessing a persons care needs, developing a care plan to meet their assessed needs - Liaising with the Local Authority to ensure all eligibility decisions are framework compliant - Ensuring out patients are safe and their needs are being met - Supporting patient and their representatives through the CHC, Fast track or FNC assessment process with clear high quality communication skills - Working collaboratively with the Personal Health Budget team to support a personalised approach to care package development - Supporting with the training and development of the CHC Framework across Kent and Medway ### Detailed job description and main responsibilities As a community Trust, many of our roles require individuals to be able to drive to perform their duties. Should you require workplace adjustments under the Equality Act 2010, we are committed to supporting you to meet the mobility requirements of the role wherever possible. ### Knowledge and Skills **Essential** - You will need to be able to be able to use IT systems and databases - You will have knowledge of up to date clinical practice by using up- to- date skills and adhering to evidence based procedures and clinical effectiveness frameworks.
