### Job overview

Please note: at this stage, the role is open only to colleagues currently employed within Kent and Medway NHS Trusts, the Kent and Medway Integrated Care Board (ICB) and Medway Community Healthcare. Nursing vacancies are also open to Student Nurses and Newly Qualified Nurses.

The All Age Continuing Care leads with the assessment, case management and oversight of the NHS National Framework for Continuing Healthcare (CHC) and Funded Nursing Care (FNC) across Kent and Medway. Assessing, developing and overseeing personalised care package for those with complex or long-term conditions both in their own homes and in care homes. Our teams work closely with the Local Authority, Primary Care Networks and system partners as part of a multidisciplinary system.

### Main duties of the job

- Assessing people both in care homes and their own homes in accordance with the National Framework
- Assessing a persons care needs, developing a care plan to meet their assessed needs
- Liaising with the Local Authority to ensure all eligibility decisions are framework compliant
- Ensuring out patients are safe and their needs are being met
- Supporting patient and their representatives through the CHC, Fast track or FNC assessment process with clear high quality communication skills
- Working collaboratively with the Personal Health Budget team to support a personalised approach to care package development
- Supporting with the training and development of the CHC Framework across Kent and Medway

## Person Specification

### Working Environment

**Essential**

- A full, valid UK driving licence or a full driving licence issued by an EU country (not exchanged from a non-EU country) and the ability to travel independently are essential for this role. Should you require workplace adjustments under the Equality Act 2010, we are committed to supporting you to meet the mobility requirements of the role wherever possible.

### Knowledge and Skills

**Essential**

- You will need to be able to be able to use IT systems and databases
- You will have knowledge of up to date clinical practice by using up- to- date skills and adhering to evidence based procedures and clinical effectiveness frameworks. You will fully understand the role and remit of a health care worker.

**Desirable**

- Having an awareness of developments within the NHS would be an advantage as well as an understanding of National Service Frameworks and NICE guidance.

### Qualifications, Training and Experience

**Essential**

- You will have a Level 6 Degree or equivalent qualification, or equivalent knowledge acquired through experience plus a diploma in nursing.
- You’ll need to have experience of managing complex caseloads and supporting/ mentoring others.
- You will be a registered health care professional with the Nursing and Midwifery Council
