Our service provides specialist rehabilitation interventions for patients with complex and long-term conditions, delivering high-quality, personalised care to support recovery and enable safe discharge home. Working as part of a multidisciplinary team aligned to local Primary Care Networks, you will play a key role in providing integrated, patient-centred care. As Charge Nurse, you will provide clinical leadership and operational support to the ward team, ensuring the delivery of safe, effective, and compassionate care. You will coordinate patient care, support staff development, and contribute to service improvement while promoting the highest standards of nursing practice. We are looking for a Registered Nurse with excellent leadership, communication, and clinical skills. Experience in rehabilitation, intermediate care, frailty, or caring for patients with complex needs is desirable. ### Main duties of the job - Removal of catheters - Administering medication - Changing wound dressings - Managing a caseload - Supporting in the supervision and development of all junior staff including Healthcare Assistants and students - Assessment, planning, implementation and evaluation of holistic care linked to packages of prescribed care - Development of integrated care programmes in partnership with the patient ### Detailed job description and main responsibilities As a community Trust, many of our roles require individuals to be able to drive to perform their duties.
