The Community Assessment Team (CAT) is based at the Emergency Departments across Gloucester Royal Hospital and Cheltenham General Hospital.  The teams purpose is to provide a rapid assessment of patients to prevent admission to the acute trust. We work between the hours of 8am-8pm, Monday to Friday, and 8am-6pm weekends and bank holidays.

The post holder will be responsible for assessing, planning, implementing and evaluating high-quality, holistic patient care, within the Emergency Department.

For new starters, there will be a 6-8 week supernumerary induction period and you will be allocated a mentor to support.

Main duties of the job

  • To assess a cohort of patients within the Emergency Department (ED) from presentation through to discharge.
  • Through training and development educate GHNHSFT and colleagues about available discharge pathways and community services.
  • Working with colleagues, to risk assesses suitability for discharge and ensure that arrangements are appropriate and timely.
  • To communicate information upon discharge to multidisciplinary teams, external agencies, patients and carers.
  • To work with community services, ie CATU, SPCA, GHC Rapid Response Team (RR).
  • Lead and support the wider team on those patients who require a more in depth Occupational Therapy or Physiotherapy assessment.
  • Line management responsibility as required.
