You’ll play a key role in supporting patients with complex health needs, long term conditions and palliative care, helping to improve outcomes and reduce avoidable hospital admissions. As a Case Manager, you will manage a defined caseload and work closely with patients, carers, General Practitioners, community services and other healthcare professionals.
