Based within a multidisciplinary service, you’ll play a key role in delivering high-quality care for patients with long-term conditions while helping to prevent unnecessary hospital admissions. You’ll assess, plan, deliver and review care, working closely with patients, carers and partner organisations. You’ll also support colleagues, contribute to service improvement and help maintain high standards of clinical practice across the team. ### Main duties of the job - Manage a caseload of patients with complex nursing and long-term condition needs - Assess, plan and coordinate care, ensuring patients receive safe and effective support - Work with patients and carers to promote independence and self-management - Collaborate with health and social care professionals to deliver joined-up care - Support and supervise colleagues while contributing to service improvement and best practice - Registered Nurse qualification and current professional registration - Experience delivering care for people with long-term conditions and complex nursing needs - Strong assessment, communication and care coordination skills - Ability to work independently and as part of a multidisciplinary team - Commitment to continuous professional development and high-quality patient care ### Detailed job description and main responsibilities Please see attached the job description and person specification for the main responsibilities associated to the role ### Applicant requirements You must have appropriate UK professional registration.
