### Job overview

Working with a community nursing team, you’ll provide expert care and coordination for people with complex health needs, helping them stay well and receive the right support at the right time. Based within a multidisciplinary service, you’ll play a key role in delivering high-quality care for patients with long-term conditions while helping to prevent unnecessary hospital admissions.

You’ll assess, plan, deliver and review care, working closely with patients, carers and partner organisations. You’ll also support colleagues, contribute to service improvement and help maintain high standards of clinical practice across the team.

### Main duties of the job

- Manage a caseload of patients with complex nursing and long-term condition needs
- Assess, plan and coordinate care, ensuring patients receive safe and effective support
- Work with patients and carers to promote independence and self-management
- Collaborate with health and social care professionals to deliver joined-up care
- Support and supervise colleagues while contributing to service improvement and best practice
- Registered Nurse qualification and current professional registration
- Experience delivering care for people with long-term conditions and complex nursing needs
- Strong assessment, communication and care coordination skills
- Ability to work independently and as part of a multidisciplinary team
- Commitment to continuous professional development and high-quality patient care

You must have appropriate UK professional registration.

## Person Specification

### Knowledge

**Essential**

- Current Legislation and impact on community nursing services.
- Evidence based practice
- Mentorship or evidence of supervision/teaching/team management
- Evidence of understanding of the role of Case Manager and care coordination.
- To demonstrate an understanding of proactive caseload management
- Integrated Team working
- Significant post registration experience of working at Band 5
- Experience in planning and providing care to patient with Long Term Conditions, wound management, palliative and end of life care.
- Experience of collaborative working across organisation both statutory and voluntary services
- Experience of initiating and implementing clinical or professional change and evaluation of practice
- Experience in using IT in clinical practice

**Desirable**

- Understanding of audit processes, Key Performance Indicators and Targets, CQUIN collection data and Quality Monitoring
- Understanding of research methodology and audit
- To have an understanding of the principles of restorative practice
- Developing and leading practitioners to deliver client responsive services delivering high quality nursing skills and care.
- Experience of proactive caseload management and the ability to act as a care coordinator.

### Qualifications

**Essential**

- RGN/RN/Diploma Nursing Studies
- Non-medical prescribing V100/V150 or willing to work towards within specified timeframe
- Specialist training/short courses relevant to the post or equivalent level of experience
- Evidence of ongoing continuing professional development/Portfolio

**Desirable**

- Specialist Community District Nursing post graduate diploma
