This post will be based in the RAAR (Rapid Access Acute Rehabilitation) service, a specialist service providing level 1 inpatient neuro-rehabilitation at an 8 bedded unit at St George’s Hospital. The Case Manager  will maintain adequate flow through the 8 bedded RAAR service by proactively liaising with external providers and agencies including but not limited to specialist rehabilitation providers, bed managers, community therapy teams, social services, housing services, equipment providers, and third sector providers. The post holder will work autonomously to support and co-ordinate, where necessary, the case management of individuals with highly complex health and social care needs. The post holder will be working to support, individuals and their families/carers in accessing the most appropriate care to meet their needs. ### Main duties of the job The Case Manager  will maintain adequate flow through the 8 bedded RAAR service by proactively liaising with external providers and agencies including but not limited to specialist rehabilitation providers, bed managers, community therapy teams, social services, housing services, equipment providers, and third sector providers.
