An exciting opportunity has arisen to join our Care Home Response Team as a Band 6 clinician based in Stanmore (HA7 1AT). This is a permanent full time role working 37.5 hours per week, Monday to Friday.

The Care Home Response Team provides high quality, safe, reactive and proactive support to care homes and their residents across Harrow. The post holder will manage a caseload of care homes and residents, delivering assessments, care planning, coordination, and person centred care to meet residents’ health needs.

You will work closely with care home staff, GPs, community services, and the wider multidisciplinary team to support residents and improve access to healthcare for some of the most vulnerable people within the community.

The post holder will be responsible for:

- Managing a caseload of care homes and care home residents across Harrow, delivering high quality, person centred care and clinical support.
- Completing holistic assessments, care planning, coordination, and ongoing review of residents’ healthcare needs.
- Building and maintaining strong working relationships with care home staff, GPs, community services, families, and the wider multidisciplinary team.
- Promoting effective communication and collaborative working to ensure safe and seamless care delivery.
- Demonstrating strong leadership, clinical judgement, and professional practice at all times.
- Supporting and educating care home staff through teaching, training, and shared learning opportunities.
- Maintaining accurate clinical documentation and contributing to service improvement and quality care initiatives.

## Person Specification

### Experience

**Essential**

- Evidence of recent post-registration nursing experience in community nursing
- Experience of negotiating and liaising within a multi-disciplinary team
- Experience of working with people with complex needs
- Experience of teaching others and mentoring students
- Experience of evidence-based care
- Experience of undertaking comprehensive health needs assessment and developing complex care plans
- Experience of co-ordinating and delivering care for people with long-term conditions.

### Key Attributes

**Essential**

- Able to carry out the duties of the post With or without reasonable adjustments
- Able to travel around the district (car/bike/public transport/foot)

**Desirable**

- Hold a clean UK drivers licence

### Skills and Knowledge

- Knowledge of principles of care for long term conditions, relevant NSFs and other national standards
- Knowledge of NMC Code of Professional Conduct.
- Knowledge of Personal Development Planning.
- Knowledge of current health care policy and relevance to the delivery of healthcare services

### Education/Qualificatoins

**Essential**

- RGN / RN with current registration
- Post –registration qualification in district nursing or equivalent
- Evidence of continuing professional education and training at degree level
- Mentor or practice based learning training

**Desirable**

- Advanced Physical Assessment course or equivalent
