We are excited to offer a full-time band 5-6 development post in Derwentside & Chester le street Mental health service for older people CMHT.

We are looking to appoint a NMC registered nurse to work into the care home liaison cell of the Community Mental Health team. The team can support a band 5 nurse through their competencies to allow them to develop the knowledge and skills of a band 6 nurse. If all competencies are completed the applicant will receive a permanent band 6 position within the team.

This post is not open to Newly Registered Nurses, candidates must have their NMC registration in place and successfully completed the preceptorship programme in order to be considered.

We are seeking an experienced and compassionate nurse to join our team. This role involves providing assessment, intervention, and support to older people who reside in care home placements. You will work collaboratively with families, carers, and multi-agency partners to ensure timely, safe, and effective care. Within the team we have a very varied workload, which means that every day will bring new and exciting opportunities for you to develop your skills. To support you in your development we will use regular appraisal and talent management, to identify your interests and development opportunities. As a team we will support you with both formal training and continuing professional development opportunities.

The team operates a service 09:00 - 17:00 Monday - Friday.  The team currently coverers Derwentside and Chester le Street areas of County Durham.  The successful candidate will work from Derwent Clinic on the Shotley Bridge Hospital site.

## Person Specification

### Knowledge

**Essential**

- Evidence based knowledge of best practice in caring for patients in the designated field.
- Understanding of relevant legislation (e.g. Mental Health Act, Mental Capacity Act)
- Understanding of Safeguarding and its application in practice
- Clinical Risk Assessment and Management and its application in practice
- Clinical Governance and its application in practice.
- Confident working knowledge of clinical medicines management, including administration of injections

### Experience

**Essential**

- Working with people with mental ill health / learning disabilities
- Working in a person centred therapeutic organisation
- Working collaboratively with service users and their families/carers
- Working in a multi-disciplinary team
- Working in partnership with other agencies

### Qualifications

**Essential**

- To be recognised practice assessor / mentor or work towards in an agreed timescale
- Trained Clinical Supervisor (within 6 month probationary period)
- Evidence of Continuing Professional Development
- Current professional registration with Nursing & Midwifery Council RNMH / RNLD
