We’re looking for a motivated, organised, and compassionate Care Coordinator to join our collective Integrated Community Teams with partner organisations — a dynamic group of clinicians, practitioners, and community partners all dedicated to delivering outstanding, joined‑up care.

### Main duties of the job

As a Care Coordinator, you’ll be at the heart of our multidisciplinary team (MDT), ensuring everyone stays connected and informed. Your role will include:

- Coordinating MDT meetings and streamlining communication
- Supporting proactive, personalised care planning for patients with complex needs
- Working closely with GPs, nurses, therapists, social care, and voluntary sector partners
- Helping the team deliver seamless, patient‑centred care across our neighbourhood
- Keeping systems updated and making sure the right information flows to the right people at the right time

### Skills

**Essential**

- Good communication skills to communicate effectively with customers, careers, specialist services and colleagues verbally and in writing. Good keyboard and IT skills including knowledge of Microsoft Office – Word, Excel and Outlook
- Ability to identify, coordinate and organise health and social care support within required timescales using negotiation and motivational skill to promote health. Knowledge of legislation relevant to Health and Social Care. Knowledge of safeguarding vulnerable adults’ policies and procedures
- Excellent interpersonal skills and diplomatic when dealing with sensitive matters Able to use initiative, with ability to meet conflicting deadlines, prioritise effectively and manage own workload Demonstrate an ability to undertake duties with a degree of autonomy and accountability Effective working as individual but also as part of larger team
- Willingness to undertake training to develop within the role Strong organisational skills, with ability to plan; prioritise; monitor progress; produce reports; identify gaps in services and opportunities for development
- Reliable work record. Organised, flexible and adaptive approach to managing workloads. Evidence that personal behaviour reflects Trust values Ability to travel independently between Trust sites in a timely and efficient manner as required in post

- Experience of computerised record systems and data storage / retrieval / analysis More than 3 years’ experience of working in health, social care or information and advice, in direct contact with people, families and carers in a paid or voluntary capacity

### Qualifications

**Essential**

- Qualifications Good level of general education including A levels and GCSE or equivalents including functional skills level 2 Evidence of continued professional development to NVQ level 4 in relevant subject or equivalent level of knowledge and experience
- Two-day coaching or motivational interviewing course and the elearning below or complete (or working towards) a PCI accredited course for care coordinators. Personalised care and support planning Shared decision making To have completed or working towards a portfolio of CPD underpinned by the competencies outlined in Competency framework for care co-ordinators

- ECDL or alternative IT qualification Healthy weight coach training Person centred approaches
