The post is based at Local community settings in Waltham Forest and the post holder will work closely with our exercise physiologists based at Royal London Hospital and Tower Hamlets community service. You will be required to work at different trust sites occasionally. We are seeking an enthusiastic individual, who is flexible, confident, and able to work independently within a multi-professional team. The key aspects of the post include delivery and evaluation of a comprehensive Cardiac Rehabilitation service. This post offers you the opportunity to:  Work in a vibrant multi-disciplinary environment within an innovative team. Although this is a Full-Time post there is potential for an additional 30hrs Part Time available. ### Main duties of the job To provide exercise prescription for cardiovascular endurance, exercise and resistance training within a cardiac rehabilitation setting. To Identify, develop and implement appropriate monitoring systems to enable comprehensive service evaluation of the physical activity / exercise component. Engage in effective partnership working to promote quality, continuity of care and a cost-effective service. To maintain a high standard of care, taking charge of own patient caseload as required.
