### Job overview

The post holder will be the point of contact for referrals into the secondary mental health team and, as such, will demonstrate a high standard of ability in communicating with a wide range of clinical and non-clinical professionals within the criminal justice team.

Effective communication is essential. To utilise nursing skills and resources to provide a high quality service to all clients and carers along with all professionals within multi-agency network.

### Main duties of the job

To develop planned programmes of care to promote health gains and maximize independence in all types of clients within the prison community.

To undertake the duty role in processing referrals into the secondary mental health team within HMP Forest Bank.

Obtaining relevant clinical information from external agencies to inform the prison mental health team in establishing the level of care need required.

Risk assessments clear and precise documentation in accordance with Trust and national professional policies and guidelines.

Coordinating the process of referrals to external care settings in preparation for transfer under the mental health act and also managing requests from care settings to remit service users back to prison custody.
