### Job overview

The Cancer Education Programme Facilitator role is to support the improvement in the quality of cancer care provided by health and care professionals across Surrey and Sussex. The role includes facilitating and delivering training on cancer from prevention and screening, through treatment and care to living with and beyond cancer.

### Main duties of the job

The post holders are expected to be registered health professionals (nurse or allied health professional) with long term conditions (LTCs) expertise. A background in clinical education would be a significant advantage.  Cancer knowledge is desirable; however, it can be achieved by building relationships with local Trust Clinical Nurse Specialists, multi-disciplinary teams (MDTs) in secondary care as well as learning and development.

The post holder will work closely with existing teams in the localities including Cancer Clinical Leads, Cancer Commissioners and Cancer Alliance Clinical Leads to ensure clear definition of roles and defined locally agreed pathways. They will develop systems and processes, to enable the pathway to be integrated and ensure the role is embedded, complementing existing service provision.

The post holder will work independently to facilitate and inform the interface of cancer pathways between health and social care and support the implementation of national/local guidance. The post holder will also identify and highlight any gaps in service provision.

### Detailed job description and main responsibilities

The post holder will:

- Facilitate and enable the education of health and care professionals on the agenda such as prevention, screening, early diagnosis, treatment and care and cancer as a long-term condition.
- Deliver patient centred, self-management support and education as necessary, including how to self-assess to promote a strong focus on health and wellbeing to improve health outcomes.
- Enhance the knowledge and skills of primary, secondary, community and social care health teams in providing care to cancer patients. This is likely to include early diagnosis, the cancer care pathway, symptom control, oncological emergencies and support and end-of-life care using other professionals as appropriate e.g., Local CNS’ and Specialist Palliative Care team.
- Enhance and improve communication between all providers, with particular attention given to the interfaces between primary care, secondary care, community care, out-of-hours (OOH) and social care to promote a seamless service and positive patient experience.
- Promote holistic needs assessment (HNA) and care planning across the care pathway to ensure that people’s physical, emotional and social needs are met in a timely and appropriate way.
- Work collaboratively with local partners and stakeholders to facilitate and inform integration of cancer pathways to ensure there are effective ways for patients and their carer(s) to navigate the complex health, public health and social care system.
- Proactively identify gaps in service provision and opportunities for redesign in line with outcomes for people living with cancer, including promoting recognised tools and models of good practice. Ensure that patients are involved in service design and development, learning from their experiences and co-producing solutions.
- Promote evaluation of services and undertake audit tools and models of good practices.
- Support primary care to provide evidence-based care for people affected by cancer who have completed or are undergoing treatment.
- Work with GP practices across localities to support them to implement national guidance around identifying patients’ healthcare needs and encouraging people to self-manage where appropriate.
- Encourage practices to support the patient and carer to tell their story and use the narrative to influence redesigning the local care pathway.
- Support practices to set up robust processes around Cancer Care Reviews (CCR) in line with the national Quality and Outcomes Framework (QOF) guidance in order to:

- Work with local care teams to ensure patients are:

- Support knowledge sharing of Health and Wellbeing supportive information for people living with and beyond cancer to health and care professionals.
- Evaluation the impact of the education and training events delivered as part of the programme, providing the Cancer Education Programme Manager with data and case studies as required.
- forge links with the Surrey and Sussex Cancer Alliance, Frimley Health Integrated Care System, Surrey Heartlands Health and Care Partnership and Sussex Health and Care.
- Network with other professionals from other areas national, regionally and locally to share good practice and embed learning.
- To establish and maintain highly effective communication routes in line with the needs of the diverse range of Surrey and Sussex Cancer Alliance stakeholder interests.
- Provide specialist advice and guidance within the Cancer Alliance portfolio.
- Motivate, persuade and influence multiple stakeholders and partners to ensure the timely completion of work programme across Surrey and Sussex, being confident to challenge as appropriate.
- Able to manage and deliver to deadline, within resources.
- Authorised signatory for small or large payments in relation to project. Orders stock, stationery, or training materials.

## Person Specification

### Qualifications

**Essential**

- General Nurse or Allied Health Professional with current professional registration.
- First level degree in related subject, or equivalent

**Desirable**

- Recognised teaching qualification

### Skills, Knowledge & Education

**Essential**

- Experience of working within an educational team and developing and delivering training
- Evidence of continued professional development acquired through training and experience to Masters' level or equivalent
- Experience of working with people with long term conditions

**Desirable**

- Experience of working in a multi-disciplinary team
- Knowledge, understanding and experience of cancer care and treatment.
- Coaching/CBT/ motivational interviewing training or experience
