- University Hospitals of North Midlands NHS Trust, inclusive of Black Country Procurement Group and North Midlands and Cheshire Pathology Service.
- Staffordshire and Stoke-on-Trent Integrated Care Board

To offer needs-led support and signposting to families and children within the service. The care navigator will ensure that the children and families are well while waiting for their assessments by signposting parents to further support. To also liaise closely with the internal and wider multidisciplinary teams to ensure the family and young person have the correct support in place. To work together with other members of the MDT.

This post is based at The Bridge, Stafford, but will include some potential for home visits to locations across South Staffordshire.

### Main duties of the job

To provide guidance to Family carers in order for them to access timely support and understanding of the assessment pathway process.

Support the team to ensure that families are kept informed of the support and options available to them

Main point of contact for families with a child on the waiting list and proactively aim to offer contact as required, individualised to the needs of the child and family/carers.

To meet regularly with supervising clinician to review clinical issues, and with team leader for managerial support.

Under supervision signpost unsuitable referrals to the relevant service as necessary.

To identify issues to allow for timely individualised assessment, escalating to line manager where appropriate.

To contribute to the assessment of and adhere to risk management measures under direction of senior clinicians.

To access regular child protection supervision and work to trust safeguarding policy.

To be a trusted link with partner agencies, the Family Hubs and the CAFSPA.

To ensure information is available for children and parents/carers in a range of formats

To participate in projects as required.

Support the team to ensure that families are kept informed of the support and options available to them and supporting them to make informed choices on how they and their child or young person are supported.

Support the understanding of the assessment processes with external agencies, partner organisations and referrers.

Support children, young people and family/carers to understand the assessment pathways through the provision of information and advice and enhance connection to the local communities.

Main point of contact for families with a child or young person on the waiting list and proactively aim to offer contact as required, individualised to the needs of the child, young person and family/carers.

To meet regularly with supervising clinician to review clinical issues, and with team leader for managerial support.

To work independently with children, young people and families/carers in a variety of settings

To highlight individual’s changing needs or wider service issues affecting an individual’s care to their supervisor.

Under supervision signpost unsuitable referrals to the relevant service as necessary.

To identify issues and assist in unblocking processes to allow for timely individualised assessment, escalating to line manager where appropriate.

To act as an advocate to facilitate views and choices expressed by child and family.

To contribute to the assessment of and adhere to risk management measures under direction of senior clinicians.

To adapt in response to changing needs and circumstances of service users and carers, to show empathy and sensitivity with regards to social situations, difficult family circumstances and understanding of condition, taking into consideration cultural, linguistic, social and psychological aspects.

To access regular child protection supervision and work to trust safeguarding policy.

To actively participate and contribute to team meetings and relevant professional forums.

To attend regular meetings with the wider multidisciplinary team, as agreed in the Operational Policy.

To support parent, child and young person involvement in the continuous improvement of the Early Years Pathways.

To be a key service link with other professional groups (eg nursery staff and therapists) and other professionals who make referrals to ensure they are fully appraised of the CAMHS Early Years Pathway, how to access it and how it works.

To be a trusted link with partner agencies and with the Family Hubs and the CAFSPA.

Undertake mandatory training and other learning opportunities identified in appraisal in line with trust policy, as appropriate to the post.

To attend courses, meetings and special interest groups as identified and agreed within the team/service.

To ensure information is available for children and parents/carers in a range of formats such as information sheets, newsletters, web-based information and appropriate signposting

To ensure outcome measures are routinely used and collated appropriately.

To provide individual activity and performance data in a timely manner on request
