We are looking for a committed clinician who is: - A Registered Mental Health Nurse (RMN) with current NMC registration - Passionate about improving the wellbeing of children and young people - Skilled in communication, risk assessment, and evidence‑based practice - Able to work effectively within a multidisciplinary team - Compassionate, adaptable, and reflective - Organised, resilient, and confident with autonomy where appropriate Experience within CAMHS, community mental health, or structured therapeutic approaches (CBT, DBT, MI) is desirable but not essential. We welcome motivated practitioners who are keen to learn and grow with the service. This is a fixed term role and for those with current NHS service, the role will be offered on a secondment basis only.  Please obtain permission to be released on a secondment before submitting an application. ### Main duties of the job About the Role This is an exciting opportunity to play a key role in an important point of a young person’s care journey. As part of the Discharge Clinic, you will: - Carry out timely assessments, wellbeing checks, and neurodevelopmental screening - Deliver brief therapeutic interventions that promote discharge readiness - Act as the lead professional for allocated young people preparing for transition - Coordinate comprehensive, recovery‑focused discharge plans - Work collaboratively with families, GPs, schools, and wider health and social care partners - Support transitions to community, education, and primary care services - Facilitate individual and group‑based interventions - Maintain high‑quality care planning and clinical record‑keeping - Participate in the duty rota as required This is a pan‑Dorset post, and some travel may be required. ### Detailed job description and main responsibilities For the main duties and responsibilities for this role please read the attached job description and person specification.  When completing your application please ensure your supporting statement reflects the criteria set out in these documents by showing how your experience and skills apply to this post.
