### JOB DESCRIPTION

POST: Personal Advisor, Leaving Care & 18+ Service

LOCATION: Leaving Care & 18+ Service, 17 Wellington Park

BAND: 4

REPORTS TO: Senior Social Worker, Leaving Care & 18+ Service

RESPONSIBLE TO: Co-Director Children's Community Services

Job Summary / Main Purpose

To support young people who are leaving care by fulfilling Personal Advisor duties to a satisfactory standard in accordance with statutory requirements and the policies and procedures of the Trust.

Main Duties / Responsibilities

For each of the following, the postholder will;

Provide advice, including practical advice and support to young people Leaving Care aged 16 and 17 &18+ Services as agreed in the Pathway Plan.

Offer support in the form of mentoring to and advocacy on behalf, of the young person, where appropriate.

Participate in assessing the needs of young people leaving care and contributing to preparation of pathway plans.

Participate in reviews of the pathway plan and request revisions to the plan where appropriate.

Liaise with the Trust in the implementation of the pathway plan as required.

Coordinate the provision of services and to take reasonable steps to ensure that young people make use of such provisions.

Stay in touch with young people and keep informed about their progress and wellbeing which will require the ability to work unsocial hours.

Support young peoples' participation in pathway planning and help them to represent their own wishes and views.

Maintain written records of contact with young people in accordance with HSS Trusts policy and procedures.

Participate in team meetings, regular supervision meetings and recognised training opportunities as agreed with the line manager.

Contribute to the development and review of the personal advisor service.

Environmental Cleaning Strategy

The Trusts Environmental Cleaning Strategy recognises the key principle that "Cleanliness matters is everyone's responsibility, not just the cleaners" Whilst there are staff employed who are responsible for cleaning services, all Trust staff have a responsibility to ensure a clean, comfortable, safe environment for patients, clients, residents, visitors, staff and members of the general public.

Infection Prevention and Control

The Belfast Trust is committed to reducing Healthcare associated infections (HCAIs) and all staff have a part to play in making this happen. Staff must comply with all policies in relation to Infection Prevention and Control and with ongoing reduction strategies. Standard Infection Prevention and Control Precautions must be used at all times to ensure the safety of patients and staff.

This includes:-

Cleaning hands either with soap and water or a hand sanitiser at the appropriate times (WHO '5 moments');

Using the correct '7 step' hand hygiene technique;

Being 'bare below the elbows' when in a clinical environment;

Following Trust policies and the Regional Infection Control Manual (found on intranet);

Wearing the correct Personal Protective Equipment (PPE);

Ensuring correct handling and disposal of waste (including sharps) and laundry;

Ensuring all medical devices (equipment) are decontaminated appropriately i.e. cleaned, disinfected and/or sterilised;

Ensuring compliance with High Impact Interventions.

Staff members are expected to involve patients, clients, carers and the wider community were relevant, in developing, planning and delivering our services in a meaningful and effective way, as part of the Trust's ongoing commitment to Personal Public Involvement (PPI).

### PERSONNEL SPECIFICATION

5 GCSE's at Grade A-C / 9-4 including English language or equivalent.

Minimum of two years' direct and recent experience of engaging with adolescent young people, on an individual basis, either in a paid or voluntary capacity.

Experience and understanding of working with others, e.g. parents, GPs, teachers, youth clubs, police etc to meet the needs of young people.

Full driving licence and access to a car.

'Where disability prohibits driving, this criteria will be waived if the applicant is able to organise suitable alternative arrangements.'

Shortlisting by Application Form

Demonstrate an understanding of the challenges facing young people today, for example, peer pressure, unemployment, drugs, alcohol.

Demonstrates an understanding of the issues facing young people leaving care, for example, separation, loss, loneliness, independent living etc.

Has the ability to listen to and respond appropriately and sensitively to the wishes and feelings of young people.

Ability to develop and maintain positive working relationships with young people, carers, families and other agencies.

Demonstrates an ability to work on own initiative and / or as part of a team.

Has the ability to make effective use of supervision and training and to access support and advice as required.

Empathy with young people.

Flexible and able to work unsociable hours.

Ability to prioritise and manage competing demands.

Ability to keep written records of contact.

Interview

### NOTE

Where educational/professional qualifications form part of the criteria you will be required, if shortlisted for interview, to produce original certificates and one photocopy of same issued by the appropriate authority. Only those certificates relevant to the shortlisting criteria should be produced. If educational certificates are not available an original letter and photocopy of same detailing examination results from your school or college will be accepted as an alternative.

If successful you will be required to produce documentary evidence that you are legally entitled to live and work in the United Kingdom. This documentation can be a P45, Payslip, National Insurance Card or a Birth Certificate confirming birth in the United Kingdom or the Republic of Ireland.
