### JOB DESCRIPTION

POST: Paediatric Nurse with a Specialist Interest in Endocrinology

LOCATION: RBHSC

REPORTS TO: Assistant Service Manager

RESPONSIBLE TO: Children’s Service Manager

Job Summary / Main Purpose

The post holder will be a highly specialist resource providing clinical expertise in care, advice and support within the specialist area of endocrinology. They will lead and work within the multi-disciplinary team in order to provide a seamless, coordinated approach to person-centred care, across the clinical pathway, and timely onward referral to other professionals and agencies as appropriate.

The post holder will lead in the provision of expert advice in developing, implementing and monitoring policies, procedures and protocols, which meet national, regional and local requirements. They will proactively advance the modernisation agenda of the endocrine service for patients and the development of the nursing service to achieve patient centered care.

He/ she will be responsible for the management of a defined caseload of children and their families. They will be required to carry out dynamic endocrine investigations and home visits to promote confidence and competence in patient self-management techniques, liaising closely with the primary health care team.

Main Duties / Responsibilities

For each of the following, the postholder will

Clinical Practice

- Act as a key worker for children and young people within the endocrine service offering an accessible point of contact across the patient journey.
- Within a nurse provided service, undertake a person- centred holistic approach to assess the physical, emotional, psychological, social and spiritual needs and the development of appropriate plans of care and appropriate onward referral.
- Plan, agree and communicate specialised programmes of care, in partnership with patients/ clients, families and their carers, to support family access to the relevant care pathway.
- Act as a resource to all disciplines involved in the delivery of care to children and young people within the endocrine service.
- Plan, deliver and evaluate nurse led clinics, utilising advanced nursing practice skills such as physical health assessment and the prescription and administration of medication.
- Play a key role in the multi-disciplinary team, including contributing to service improvement within the team.
- Work in collaboration with the wider health and social care team to co-ordinate the delivery of high quality patient centred care.
- Provide professional and clinical leadership that facilitates professional decision making and effective team working within the endocrine service.
- Demonstrate clinical expertise in relation to paediatric endocrinology by role modelling child and family centred care and advanced professional standards of leadership, supervision and clinical management.
- Be responsible for the organisation and management of own caseload, including liaison where appropriate with other agencies, and the provision of specialist advice and treatment recommendations within the clinical area.
- Provide expert advice to children, young people and their families with endocrine conditions to enable them to adjust to living with chronic illness and to achieve optimum quality of life.
- Role model excellent advanced communication skills and expertise in patient education and information giving.
- Demonstrate skills in conflict resolution and competent negotiation skills when dealing with difficult or challenging situations/behaviour.
- Carry out dynamic endocrine tests such as Insulin Tolerance test and Glucagon tests and others to agreed protocols.
- Coordinate the follow up of newborns with positive TSH results identified through the Neonatal Screening Programme.

Leadership & Management

- Be aware of emerging evidence, new mandates and advances in clinical practice for this patient group and ensure that these are incorporated into the service delivered.
- Engage regionally and nationally in the development and implementation of multi-professional standards and clinical management guidelines for the care of children and young people with endocrine conditions.
- Develop and maintain effective communication networks across service and professional boundaries and ensure effective liaison between different service providers across care interfaces.
- Manage and prioritise patient/client care as appropriate to service need, advocating timely access and review which meets clinical guidelines and statutory responsibilities.
- Develop and lead the service for young people with Gender Dysphoria and to provide a high quality evidence based patient centred service.
- Liaise with the Child and Adolescent Mental Health Service (CAMHS) AND Knowing Our Identity (KOI) teams to ensure that the young person and their families are provided support and counselling.
- Lead in the provision of transition clinics for young people moving to the adult service and ensure a smooth transition.
- Attend meetings for all Endocrine, Gender Identity Dysphoria Nurses throughout Northern Ireland and the UK.
- Lead in the development of a bone service for children and young people with endocrine conditions.
- Appropriately delegate, supervise and support others within the scope of each individual’s role and in line with the NMC Code (2015), retaining responsibility and accountability for results.
- Take a lead role, in collaboration with others and with the network, in the development of policies, guidelines and protocols related to the care of endocrine patients and their carers and family.
- Lead in the analysis of areas of need, working collaboratively to identify gaps in service provision, providing leadership in the development and management of services that meet the needs of children and young people with endocrine conditions.
- Lead on the patient experience component of the care of children and young people with endocrine conditions including engaging in the development of and maintenance of regional information pathways.
- Co-ordinate any patient and user involvement initiatives concerning this group and any user or service feedback mechanisms regarding service improvements.
- Support the implementation of any regional standards regarding the care of children and young people with endocrine conditions.
- Ensure a safe environment for patient care is underpinned by best practice guidelines and Health and Safety policies and guidelines, including the use of risk assessments and the development of action plans to mitigate/eliminate identified risks.
- Ensure all strategies, policies and protocols relating to infection control are implemented and monitored, and that appropriate and timely action is taken in response to ensuring the prevention and control of infection.
- Manage and report accidents, incidents or hazards according to Trust policy and procedure.
- Locally resolve any concerns or complaints of patients or others where possible by early intervention, and to investigate formal written complaints, forwarding a response to the Assistant Service Manager in the timeframe required. To work with the Assistant Service Manager on action plans for improvement and lessons learned.
- Monitor and manage expenditure against allocated budgets in collaboration with the Assistant Service Manager.
- Participate in the recruitment and selection of staff up to and including Band 6 and their subsequent induction and initiation to the workplace.
- Have a flexible approach to own working hours in direct relation to service delivery needs.
- Be present at and actively participate in meetings as appropriate, disseminating any organisational/nursing information to others, and to represent nursing views as necessary at these meetings.
- Chair meetings as appropriate promoting open communication and participation.
- Support the development, delivery and evaluation of structured patient education, working in collaboration with the multidisciplinary team and colleagues as appropriate.

Education and Development

- Undertake continuous professional development, maintain clinical competency and knowledge of emerging evidence and advances in clinical practice, ensuring that they are disseminated and shared with healthcare professionals and incorporated into the service delivered.
- Work in collaboration with others to formally review the outcomes of the services for patients with endocrine conditions and their families, carers, friends or relatives and ensure that any issues identified are addressed appropriately.
- Collaborate with the Assistant Service Manager, the Nursing Development Lead and others to identify learning needs and required actions consistent with service priorities which reflects the future requirements of the service.
- Promote, lead and enhance evidence based practice in liaison with the Nursing Development Lead and others, to create a learning environment based on the principles of lifelong learning.
- Provide the specialist knowledge of your role as a development resource for your team.
- Develop a culture, which encourages and supports staff in reflective practice using reflective practice strategies.
- Provide formal and informal training and education on issues related to the care of children and young people with endocrine conditions working in collaboration with the multidisciplinary team and colleagues as appropriate.
- Demonstrate commitment to lifelong learning by engaging in on going personal and professional development.
- Support supervision and mentorship for pre and post registration students, registered nurses and the health care team as appropriate.
- Participate in NMC revalidation processes including supervision, reflective practice and self-evaluation to improve care and practice.

Research

- Promote a culture of research and evidence based practice within the service to support continuous improvement activities.
- Lead in appropriate local and national nursing and multi-professional research, clinical practice development projects and programmes aimed at contributing to the outcomes and experiences of children and young people with endocrine conditions and their families, carers, friends or relatives.
- Disseminate relevant research findings in the promotion of best practice and service improvement.
- Lead in appropriate audit, peer review sessions and the use of evidence based practice specific to the care of children and young people with endocrine conditions to enhance patient care and contribute to the implementation of findings.
- Lead in developing, implementing and monitoring policies, procedures and protocols.
- Lead in analysing health and care technologies providing feedback to inform selection and use within the endocrine service.

The Belfast Trust is committed to reducing Healthcare associated infections (HCAIs) and all staff have a part to play in making this happen.
