### JOB DESCRIPTION

POST: Ward clerk

LOCATION: BHSCT Trustwide

GRADE: Band 3

REPORTS TO: Ward Clerk Team Leader

RESPONSIBLE TO: Ward Clerk Co-Ordinator, Unscheduled & Acute Care

### JOB SUMMARY/MAIN PURPOSE

The post holder will provide a high level of clerical support at ward level to a multidisciplinary team and work as an integral part of the ward team. The post holder will be expected to utilise EPIC to input data and provide information to appropriate professional staff at ward level.

They will be expected to work under pressure and without direct supervision. Participate in general office duties in accordance with local Policies & Procedures.

As the post holder, he/she will be the main point of contact to handle calls from GP’s, Patients & relatives taking the responsibility to monitor the calls and deal with them appropriately in relation to their role at ward level.

Main Duties/Responsibilities

Responsible for ensuring that all new admissions elective & emergency are registered appropriately on EPIC accurately and timely

Triage new admissions to the ward every day by sourcing the appropriate Consultant/Speciality using triage grids/Microsoft teams and update EPIC accurately & timely

Responsible for maintaining & updating patient demographic details, adding/revising on EPIC as appropriate

Manage any ward diaries, liaise with Theatre Schedulers/waiting list office to source and prepare patient records for planned admission

Request and print letters, documents, labels & reports using EPIC to assist medical/nursing staff with patient history & patient tracking.

Arrange and co-ordinate interpreters for inpatients on the ward using electronic systems and accurately record receipts

Monitor and take action on delayed discharge information to ensure timeliness/accuracy of discharge coding in line with the agreed Delayed Transfer of Care from General Acute Sites

Ensure all documentation & records are appropriate and up to date when the patient is admitted to the ward

Scan and upload patient documentation to the patient record on EPIC as required

On discharge order ambulance transport if required and ensure the timely completion of ward discharge note.

Liaise with Consultants, medical secretaries, and nurse specialists to ensure that any ward reviews are highlighted

Contribute to the prompt answering of phone calls in a courteous and pleasant manner.

Liaise with client, staff and departments internal and external to the Trust ensuring the delivery of a full and efficient service.

Keep information flowing between hospital staff, patients, family members & other departments as appropriate.

Operate the Inter Hospital Transfer Policy for patient’s medical notes if required.

Maintain a clear, tidy workstation area in compliance with Data Protection

Able to manage and deal with competing demands as work pattern becomes unpredictable due to interruptions from e-mails, telephone calls and direct patient contact.

Maintain standards of confidentiality regarding all information held in respect of Service users and professional staff

Report faults to Estates Department or IT and telephony.

Order transport for patients being discharged, transferred or attending other hospital sites for investigations

Provide emergency cover for colleagues on a short term basis

Adhering to General Health and Safety policies and procedures including the transportation & storage of patient records.

Participate in annual review process to enhance Service Delivery & enhance own learning & development.

Attend relevant training sessions as required for the job role.

Undertake any other duties appropriate to the band that may be assigned from time to time to meet changing circumstances

The Senior Manager is responsible to the Co-Director for Unscheduled and Acute Care for all records held, created or used as part of their business including corporate and administrative records whether paper-based or electronic and also including emails.

All such records are public records and are accessible to the general public, with limited exceptions, under the Freedom of Information Act 2000, the Environment Information Regulations 2004 and the Data Protection Act 1998.
