This is an exciting opportunity to play a key role in supporting safe, timely and effective patient care at the front door, helping to identify the right pathways for patients and supporting care closer to home wherever possible. Working closely with acute, community, local authority, mental health and voluntary sector partners, the post holder will provide senior clinical assessment, decision-making and leadership to support admission avoidance, early supported discharge and improved patient flow across the health and care system. ### Main duties of the job The main duties of the role will include undertaking senior clinical assessment and triage of patients with acute, complex or changing needs; supporting timely clinical decision-making; and identifying appropriate community-based alternatives to hospital admission. The post holder will work collaboratively with multidisciplinary and multi-agency colleagues to support urgent community response, virtual ward, rehabilitation, specialist community services and safe discharge pathways. They will contribute to board rounds, MDT discussions and care planning, provide clinical advice and support to colleagues, maintain accurate clinical records and promote person-centred, evidence-based care that helps reduce delays, unnecessary admissions and length of stay. ### Detailed job description and main responsibilities Please see the Job Description and Person Specification attached for further information on the role.
