Patient Pathway Coordinator (Band 4) – Booked Admissions

The Booked Admissions Patient Pathway Coordinator is responsible for managing elective patient pathways from decision to admit through to treatment, ensuring patients are scheduled within clinically appropriate and RTT targets. The post holder will coordinate theatre and inpatient bookings, maintain accurate patient administration system records, monitor waiting lists, and proactively resolve pathway issues to support timely patient care. Working closely with clinicians, operational teams and patients, the role requires excellent organisational skills, attention to detail, and the ability to manage competing priorities in a fast-paced environment while delivering a high standard of customer service.

Key duites include:

- Monitor and review patient care and/or treatment progress using the Patient Tracking List.
- Identify from the Tracking List the next steps in the patient care pathway in line with clinical need (peer review).
- Receive patient referrals and administer in line with timings and requirements as set out in Trust and National policies
- Provide day-to-day supervision and oversight of duties for the department’s administrators.
- Providing support for day-to-day queries from administrators.
- Responsible for collecting and updating patient information on the relevant databases in line with local policy,

### Applicant requirements
