### Main duties of the job

The post holder will:

- Deliver an expert, high-quality primary care service within custodial settings
- Work autonomously to assess, plan, implement and evaluate patient care
- Manage complex and long-term conditions, urgent presentations and clinical prioritisation
- Provide clinical leadership within a multidisciplinary and integrated healthcare team
- Promote health equality and inclusion health approaches within the prison population
- Support the ongoing development of safe, effective and responsive healthcare services

Additional Bank Requirement:

- Work flexibly across sites with minimal supervision and adapt quickly to different teams and environments
- Maintain clinical competence to work safely with limited local induction

### Detailed job description and main responsibilities

Clinical

- Undertake comprehensive and advanced patient assessments
- Manage patients with complex, multiple and long-term conditions
- Lead primary care clinics (e.g. LTC management, triage, urgent care)
- Deliver evidence-based interventions and evaluate outcomes
- Administer and manage medications safely in line with policy
- Identify deterioration and respond appropriately within a secure environment
- Make appropriate referrals and ensure continuity of care

Leadership and Coordination

- Act as shift lead / nurse in charge where required
- Supervise, support and develop Band 5 nurses and wider team members
- Provide clinical guidance and decision-making support
- Coordinate care delivery and prioritise workload across the service
- Contribute to maintaining safe staffing and patient flow

Administrative

- Maintain accurate, contemporaneous clinical records in line with CNWL policy, demonstrating high standards of record keeping and documentation
- Complete prison-specific documentation (e.g. incident reports, security intelligence where appropriate)
- Support data collection, audits and service reporting

Education and Development

- Complete all mandatory training.
- Support junior staff and peers, both formally and informally, in their development using encouragement, informal teaching and coaching activities.
- Work with your Senior Managers and wider multidisciplinary team to provide appropriate learning experiences for colleagues, students and others allocated to the team, facilitating new skills and knowledge required to meet changing needs.

- Work within restrictive regimes and adapt to operational constraints

Working Conditions

- Staff may be exposed to distressing or emotional circumstances such is information regarding patient’s criminal behaviour or occasionally exposed to distressing episodes of self harm.
- Staff will be required to concentrate for periods of time where the work is unpredictable.
