### Job overview

We are looking for an experienced and highly motivated nurse who is passionate about palliative and end of life care to join the Children’s Specialist Palliative Care Team. Alongside the Clinical Lead you will play an active part in clinical practice, education, research/audit and the implementation of national guidelines. The successful applicant will work as part of a well-established dynamic team who provide specialist advice and support to children, young people and their families and all members of the health care teams involved in the child/young person’s care.  The team consists of a specialist palliative care consultant, senior palliative care nurses, senior family support worker and nurse associates. We work very closely with the Children’s Community Nursing and Rapid Response Teams and the acute settings to ensure a high standard of care is offered to children/young people and their families throughout their palliative care journey.  The team support children and young people and their families with all aspects of palliative, end of life care and bereavement support. The team offer a 24/7 service which enables timely symptom management and offers family choice around care for place of death.  It is desirable that you will hold the Independent Non Medical Prescribing qualification and have a good understanding of symptom control. Advanced communication is essential to this role.  Full time/part time/flexible hours of working will be considered at interview.

### Main duties of the job

- Give encouragement, support, empathy, time and reassurance and advice to patients, families and carers to enable them to explore and express feelings and issues of concern.  - Working at an enhanced practice standard, co-ordinates and manages a defined case load of children facilitating the highest possible standards of individualized care  - Demonstrate advanced practice in holistic assessment, development and evaluation of individual specialist nursing care programmes involving patients, families and carers as appropriate.  - Facilitate the management and support of palliative care patients to ensure the pathway is seamless and of a high quality.  - Demonstrate advanced knowledge as an expert practitioner in palliative care by use of innovative practice and skills.  - To undertake non-medical nurse prescribing and to act as a nurse prescriber within the boundaries of qualifications and professional guidelines in accordance with legislation policies and procedures  - Identify and apply innovative care and practice to ensure best clinical outcomes to benefit the patients, families, carers and organisational needs.  - Analyse complex situations, which may be detrimental to a child’s physical or psychological well-being and develop action plans to improve the quality of patient care in line with Trust and local agency policies e.g. child protection issues.
