An exciting opportunity has arisen to work as a Senior Heart Failure Specialist Nurse in the Sirona Community Heart Failure Service.

We are looking for a motivated and passionate clinician to join our team.

Our community-based specialist service works to improve outcomes for patients living with heart failure. By working as part of a wider Multi-Disciplinary Team we strive to deliver best evidence care that is individualised for our patients. We work closely with our partners in Primary and Secondary Care, providing early treatment intervention and supporting the transition from hospital to community management. The service also offers an advice and guidance to other Health Care Professionals across Bristol, North Somerset & South Gloucestershire.

There will be a requirement to work across BNSSG to meet service delivery, but we look to deliver clinical caseload elements of the service within geographical locations with bases currently within Bristol and North Somerset and South Gloucestershire.

This role is open to clinicians currently working at Band 7 level looking for a change in role, or as a career development opportunity for Band 6 clinicians. All applicants should have experience working within cardiology or heart failure pathways.

### Main duties of the job

- To work in partnership with individuals, families and carers, using a range of assessment methods as appropriate
- To use specialty expertise and decision-making skills to inform clinical reasoning approaches when dealing with differentiated and undifferentiated individual presentations and complex situations
- To Provide clinical leadership, expertise, education and clinical supervision to immediate team members and other individuals across the organisation and system
- To develop and implement policies, protocols, standards of care and guidelines in line with national and local policy within the team and specialty policies for the wider organisation.
- To be able to communicate complex patient related information effectively to ensure collaborative working with MDT members and with MDT colleagues across health and social care sectors to ensure the delivery of a co-ordinated multidisciplinary service
- To manage a clinical caseload of patients within the heart failure service pathway

### Skills, experience and knowledge

**Essential**

- Enhanced Clinical skills and experience of working with people with heart failure
- Able to demonstrate leadership skills, building effective working relationships with staff and management at all levels within the organisation and external environments
- Exceptionally efficient, with excellent prioritisation, time management and organisational skills and the ability to work flexibly and on a number of projects at one time to tight deadlines and at time under pressure
- Ability to act on professional judgement about when to seek help, demonstrating critical reflection on own practice, self-awareness, emotional intelligence, and openness to change
- Experience of community and multidisciplinary team working
- Understanding of the implication of cultural difference for service delivery
- Up to date knowledge of heart failure management, national and local guidelines and evidence base via current research and journals

**Desirable**

- Be actively involved in professional clinical groups, such as; Professional Groups, Clinical Interest Groups, Peer Review Groups and other professional development activities.

### Qualifications and training required

**Essential**

- Current professional registration, such as NMC as a 1st level Registered or HCPC registration
- Successful completion of a relevant post-graduate L6 (or higher) course or appropriate equivalent modules, such as disease specific qualifications, Physical assessment and clinical reasoning.
- Evidence of continuous personal and professional development and ability to study at Masters Level.
- Willingness to undertake training required for the role such as Independent Prescribing if not already completed or PADRAP/PACR
- Hold a UK Driving Licence and have access to a vehicle during working hours.

**Desirable**

- Master’s level clinical qualification or working towards achieving this.
- Coaching / Teaching and Assessing qualification / experience
